1 The "Telephone Call Template" is actually a small set of templates used to document communications between a practice and it's patients. Accessing the Telephone Call Template The "Telephone Call Template" is a stand alone template and can be accessed from the "Template Module" located at the bottom of the Patient's History panel. The Telephone call template can also be accessed from the following locations: Patient Information bar Provider Test Action Template For the purpose of this lesson, we will access the Telephone Call Template from the Template Module. 2 Select "Telephone Call MG" from the "Preferred" template list then, click "OK". Note: if the Telephone Call Template is not in your Preferred Template list, click the "All" radio button to see the full list of templates. The "Telephone Call Template" is now open. Specialty To select the Specialty for your practice/doctors office, click on the drop down arrow next to "Specialty". Note: the red text indicates a required field. 3 Visit Type After selecting the Specialty. The "Visit Types" picklist automatically opens. Select "Patient Communication" from the picklist then, click "OK". Note: The Specialty and Visit Type will determine what linked tabs will be available on the Telephone Call Template. (i.e. Telephone Call, Provider Test Action, Document Library, etc.) Patient Contact Info By clicking the "Patient Contact Info" button. The patient's contact information as well as "Patient Relationship/Support Role" and "Pharmacy Information" will be visible. View Scheduled Appointments To view past, present, and future appointments for the patient, click on the "View Scheduled Appointments" button. Note: Appointments can not be scheduled/rescheduled in the View Scheduled Appointments window. Telephone Call Template KBM 8.3, July 2, 2014 Page 3

4 Telephone Call Summary Telephone Summary template popup displays the history of communications with or regarding the patient. The summary can be launched by clicking the "Telephone Call Summary" button. PCP If the patient's Primary Care Provider has been recorded in Practice Management, the name will appear in the "PCP" field. Note: the PCP field can not be modified from the Telephone Call Template. Panel Controls Use the "Panel Controls" to Toggle or Cycle the Telephone Call panels. Communication When taking a call, it is important to find out the nature of the call. Click the drop down arrow to the right to expand the "Communications" panel. Telephone Call Template KBM 8.3, July 2, 2014 Page 4

5 Based on the nature of the call, select the "Communication Type". We will select the "Medical Question" type in this illustration. Call Information The "Medical Question" template is open. Enter the Contact type, Urgency of the request, and Who was Spoken to in the "Call Information" fields. Note: the Date of call and Time of call fields can be modified. Contact Information Select the "Preferred contact" method for the patient, by clicking the radio button next to the desired method. Telephone Call Template KBM 8.3, July 2, 2014 Page 5

6 Communication Record patient's "Concern(s)" as well as the "Duration of symptoms". Additional information about the patients concerns can be free texted in the "Comment" field. Note: My Phrases can be inserted into the "Comment" field. Actions The "Actions" section can be used to quickly document common actions and details, and allows staff to note when an action is completed. Tasking To assign a person to the task and to send the Telephone Call record to their Inbox, click the "Send & Close" button. Note: the Priority can be changed to High or Low, by clicking in the "Priority" field. Telephone Call Template KBM 8.3, July 2, 2014 Page 6

7 Additional Features In each of the "Communication Type" templates, there are additional features that can be used to view and or record additional information. Here is a list of some of those features below: Meds/Allergies/Chronic Probs - View patient's active medications, allergies, and problems lists. HIPAA - Used to document the rules for information disclosure agreed to by the patient. Admin Action - Enables the action taken to be associated with a specific communication. Release of Info Log (PHI) - Used to record any release of private health information. Review of Test(s) - View orders and test such as "Diagnostic studies", "Referrals", " Lab Orders" and "Lab Results". If the Telephone record has been completed, click the "Save & Close" button. Communications Grid Notice that the patient's concern is seperated from the actions in the "Communications Grid". Telephone Call Template KBM 8.3, July 2, 2014 Page 7

8 Removing Communications Communication information can be deleted from the "Communications Grid", by selecting the row of the communication then, clicking the "Remove" button. Orders Panel By expanding the "Orders Panel" order information such as Labs, Diagnostics, Office Services, Immunizations Due, Procedures, and Referrals can be viewed. Note: additional active sublinks are also available in the orders panel. (i.e. Order Management, Immunizations, Standing Orders, etc.) Protocols Patient protocol information can be viewed, by clicking the drop down arrow to expand the "Protocols" panel. Generating Telephone Call Document To generate a document from the Telephone Call Template, Click the "Preview Document" button. If the document is not to be previewed, but generated offline, click the "Offline" button to generate the document. (See Image Below) Telephone Call Template KBM 8.3, July 2, 2014 Page 8

9 Note: when generating documents offline, please be aware that there may be a delay before the document is added to the patients chart. Provider Communications Communication of exam findings and other results can be communicated and recorded, by clicking on the "Provider Communications" button.
