3 Ashland Independent School District will be referred in this document as AISD Provider will be referred to provider, service provider or bidder AISD currently has in place single mode fiber between all mentioned locations above. Our current 5 year contract with Windstream Communications ends on June 30, AISD is seeking interested bidders to continue or expand the existing fiber service from central office to each school. AISD will seek USF funding for this service. All bidders must have a Service Provider Identification Number (SPIN) on file with the Universal Service Administrative Company. Additionally, the bidder must be recognized as a Telecommunications Provider by USAC. The SPIN shall be listed on the Proposal. Cost proposals shall quantify distance, materials and cost for point-point connections from all schools to the AISD Central Office. Additionally, proposals shall assert that the bidder has included all cost for rightof-way easements, pole attachments and permits associated with completing the project. After award, AISD will not consider changes in cost or terms arising from the data link construction conditions or fees from third parties. If Provider will be using existing fiber from Windstream, the Provider must submit monthly cost of leasing the fiber to AISD. Proposals should address the following district requirements: Data speed of 1 Gbps or higher Reliability, including expected uptime and Mean Time to Repair (MTTR) Expandability Streaming Video capabilities Suitability for VoIP, including descriptions of anticipated Quality of Service (QoS) Acknowledgment should be made of the fact schools operate on an annual budget and cannot forecast financial circumstances beyond one fiscal year. Should a dire financial situation arise upon 60 day notification, the District may be released from the contract due to such adverse conditions. The bidder is encouraged to submit costs for multiple data speed options and payment options. (e.g higher install cost / lower monthly charge vs. lower install cost / higher monthly charge.) These cost options should be tabulated or otherwise listed in a clear logical format. Cost proposals shall show the following: Initial install costs On-going support and maintenance costs Cost to upgrade the infrastructure to higher bandwidth Other recurring and non-recurring charges Proposals shall provide adequate references for past projects of similar size.

4 All questions must be submitted in writing prior to 2:00 pm on Monday, January 20, Direct questions to with the subject line of District Wide Network Upgrade All proposals must be submitted in writing by sealed envelope to the attention of Cary Williams, Director of Instructional Technology, 1820 Hickman St. Ashland, KY prior to 2:00 pm on Monday, January 20, All envelopes shall be clearly labeled Sealed Bid and District-wide Network Upgrade. The District reserves the right to determine the ability of any Contractor to perform the work. All requirements of the Proposals will be reviewed in the evaluation of each bid. In determining the qualifications and responsibilities of the bidder, the District shall take into consideration the bidders skill, experience, facility, previous work standing, financial standing, capacity and ability to handle work in addition to that in progress, and quality and efficiency of construction plant and equipment to be utilized on the project. The service provider shall assume sole responsibility for all operating, maintenance, and performance functions of service for the life of the contract. The successful bidder shall be responsible for all make-ready charges, application and ongoing fees for right of way, pole rental agreements and permits throughout the life of the contract. The successful bidder will be responsible for all physical changes required by local, state, or federal agencies and must adhere to all local, state and federal codes. The service provider shall assume during the life of the contract, irrespective of physical media revisions. The service provider shall retain ownership and responsibility for all equipment and / physical plant installed as part of this contract.

5 Technical Description Section 1. Technical Specifications 1.1 Interfaces Provider shall use existing or must terminate 1 Gigabit Ethernet WAN to AISD via a 1 Gigabit Ethernet copper connection hand off into the Main Distribution Frame (MDF) at each location. 1.2 VLAN Transparency Customer must configure its own VLAN on AISD equipment between locations without coordination with provider. Provider must allow for VLAN Transparency. 1.3 Routing The service shall provide OSI layer II connectivity only. Layer III routing will be provided at the AISD Central Office. 1.4 Customer Information Provider will pass AISD information from one end of the WAN to the other without manipulating or scheduling the data. The routing and switching of the data is controlled by AISD through Customer provided switches and routers. 1.5 Custom Network The 1 Gigabit Ethernet WAN are a dedicated service and each location and will have full Gigabit available at all times. All solutions must utilize dedicated media. AISD data and bandwidth shall be kept isolated from foreign traffic. The service shall provide an Ethernet handoff into the Main Distribution Frame (MDF) at each location. Section2. Monitoring, Technical Support and Maintenance 2.1 Network Monitoring Provider should offer 24/7/365 monitoring of provider equipment 2.2 Technical Support Provider offers 24/7/365 technical support if trouble occurs and should be easily accessible via 800 number. 2.3 Escalation Provider should provide trouble ticket and reporting to keep AISD apprised of the situation. Provider should ask for two (2) contacts and phone numbers from AISD that may be contacted in the event of a problem.

6 2.4 Maintenance Scheduled maintenance should be coordinated between provider and AISD. Provider should provide a minimum of 48 hour notice for scheduled (non-emergency) maintenance. Section 3. Responsibilities 3.1 Provider will provide all entrance facilities and will demark the WAN in the building as directed by AISD. 3.2 Provider will utilize AISD conduit if available. If conduit is not available provider will install its own entrance facilities. 3.3 AISD at its own cost, shall provide collocation space and power in applicable location(s) and access to such space, including and necessary easement and building entrance rights to extend network from the public rights of way into any such location(s).
