1 2004 Specifications SPECIAL PROVISION Instructions to Bidders For this project, Item 002, Instructions to Bidders, of the Standard Specifications, is hereby amended with respect to the clauses cited below, and no other clauses or requirements of this Item are waived or changed hereby. Article 2.2. Eligibility of Bidders is supplemented by the following: Bids will not be accepted from bidders unless the bidder or the bidder's designated subcontractor(s) have been pre-qualified by the Texas Department of Transportation, Traffic Operations Division. This technical pre-qualification is in addition to all other bidder prequalification requirements set forth by the Construction Division of TxDOT. To pre-qualify, file acceptable documentation demonstrating the capability of the bidder or the bidder s designated subcontractor(s) to perform the specific categories of work that are described below which apply to this contract. The categories of work that apply to this contract are listed in the "Important Notice to Contractors" Special Provision in the contract. Category A.1.0. Pulling Fiber Optic Cable. a. Two years continuous existence offering services in the installation of fiber optic cable through a conduit system and terminating in ground boxes, field cabinets or buildings. b. Two completed projects where the personnel pulled a minimum of 5 mi. in length of fiber optic cable through an outdoor conduit system for each project. The completed fiber optic cable systems must have been in continuous satisfactory operation for a minimum of 1 yr. Category A.2.0. Splicing and Testing of Fiber Optic Cable a. Two years continuous existence offering services in the fields of fiber optic cable splicing and testing of fiber optic cable installed through a conduit system and terminating in ground boxes, field cabinets or buildings. (1) Termination of a minimum of 36 fibers within a Fiber Distribution Frame

2 (2) OTDR testing and measurement of end to end attenuation of single mode and multimode fibers. (3) System troubleshooting and maintenance. (4) Training of personnel in system maintenance. (5) Use of water-tight splice enclosures. (6) Fusion splicing of fiber optic cable which meet the tolerable db losses listed below: Mode db Loss Range Single mode 0.05 to 0.10 Multimode 0.20 to 0.30 b. Two completed projects where the personnel performed fiber optic cable splicing and terminations, system testing, system troubleshooting and maintenance during the course of the project and provided training on system maintenance. Each project must have consisted of a minimum 5 mi. of fiber optic cable. The completed fiber optic cable systems must have been in Category B.1.0. Dynamic Message Sign (DMS) Installation. a. Two years continuous existence offering services in the installation of DMS signs. b. Two completed projects consisting of a minimum of 2 signs in each project where the personnel installed and tested DMS on outdoor, permanently mounted overhead structure(s) and related sign control equipment. The completed sign system installations must have been in continuous satisfactory operation for a minimum of 1 yr. Category C.1.0. Closed Circuit Television (CCTV) Equipment Installation. a. Two years continuous existence offering services in the installation of CCTV camera systems. b. Two completed projects consisting of a minimum of 5 cameras in each project where the personnel installed and tested CCTV cameras on outdoor, permanently mounted structure(s) and related camera control and transmission equipment. The completed CCTV camera system installations must have been in continuous satisfactory operation for a minimum of 1 yr

3 this project from the supplier who furnished pre-qualification documentation. Category D.1.0. T-1 Digital Communication System Installation. video communication systems operating at a minimum of a T-1 communication rate (1.544 Megabytes/second (Mbps)). (2) Installation of communication switching equipment. (3) Integration of a network management system. (4) End to end communication system testing and maintenance. b. Two projects completed by the personnel consisting of T-1 digital communication system installations, with a minimum of 3 drop/insert nodes, operating at a minimum of a T-1 (1.544 Mbps) communication rate over transmission distances exceeding 5 mi. for each project. The completed communications system installation must have been in Category D.2.0. Synchronous Optical Network (SONET) Communication System Installation. video communication systems operating at a minimum of an OC1 communication rate (51.84 Mbps). (2) Installation and integration of digital DS1 and DS3 communication switching equipment. (3) Integration of a network management system. (4) End to end communication system testing and maintenance

Two projects completed by the personnel consisting of communication system installations with a minimum of 3 drop/insert nodes operating at a minimum of an OC1 (51.84 Mbps) communication rate over transmission distances exceeding 5 mi. for each project. The completed communications system installations must have been in Category D.3.0. Asynchronous Transfer Mode (ATM) Communication System Installation. video communication systems operating at a minimum of an OC3 communication rate (155 Mbps). (2) Installation and integration of digital DS1, DS3 and ethernet communication switching equipment. (3) Integration of a SNMP (Simple Network Management Protocol) based network management system. (4) Installation and configuration of circuit emulation for T1 and PBX circuit transmission. (5) End to end communication system testing and maintenance. (6) Configuration of control signaling via PNNI (Private Network to Network Interface) protocol. b. Two projects completed by the personnel consisting of communication system installations with a minimum of 1 ATM node operating at a minimum of an OC3 (155 Mbps) communication rate over transmission distances exceeding 5 mi. for each project. The completed communications system installations must have been in Submit completed prequalification documents by 12:00 P.M. Any documentation which does not correctly address all

5 specified items will be rejected for the reason of insufficient data and additional information must be provided. Failure to submit prequalification documentation prior to 12:00 P.M. (CST), 8 working days prior to bid opening, will be sufficient reason for not being approved for bidding. All prequalification documentation must appear on TxDOT Technical Prequalification Forms for each applicable category of work. Technical Prequalification Forms may be obtained upon written request to the above address, or by calling the office of the Engineer of Traffic at 512/ Should the bidder have approved subcontractors which meet the above requirements, and should these subcontractors be unable to complete the entire project, the successful bidder must resubmit technical pre-qualification material on alternate subcontractors for approval before the applicable category of work can be continued
