1 Welcome to Privit Profile! This document provides instructions to students on completing the Privit Profile process. The information can be completed on your mobile device, laptop, tablet or any device connected to the internet. If you need assistance with Privit Profile TM, please contact the Help Center at or visit This is REQUIRED. Spring Students by Jan 1 st. Fall Students by August 1 st. Re-Admitted Students 1. Log in to your account. 2. Update all forms. 3. Upload your immunization record if it is not currently uploaded. New/Transfer Students - Steps to complete within Privit Profile TM: 1. Register an account here: 2. Once in your account, click your name and add a profile picture one of YOU! 3. Complete all relevant information 4. Apply electronic signature 5. Join 2019 New/Transfer/Re-Admitted Team. If you are an ATHLETE also join your appropriate athletic team. 6. All Students - Upload Copy of Medical Insurance Card- front and back. 7. All Students - Upload Immunization record. This is required. Upload your copy to Privit. If you are unsure where to obtain a copy of your immunization records try the following: your high school, current college, physician s office or clinic. If you are missing any required immunizations, these can generally be taken care of at your local pharmacy or Health Department. We do not accept paper copies or faxes. You must upload these to your account. REQUIRED Immunizations for ALL STUDENTS: This information must be provided by your physician. You may attach an official copy; copies must be or have an official stamp or physician signature. D.T.P. 2 Step 1: Register an account: Register an account in your name and complete the appropriate information. Create your account by selecting or entering the following link then follow the steps below: 1. From the landing page, click Register. 2. Please register with your name, your address and create a password of your choice. When you are finished, click Sign Up. You will then be taken to the HOME page. Note: If you already have an account, skip Step 1 and log in with your credentials. Forgot your password? Use the Forgot Password link on the landing page or call the PRIVIT Support Team at Step 2: Complete All Relevant Information: 1. Complete each section by clicking the Start button to the right of the section. Step 3: Apply Electronic Signature to Each Section: 1. After each section is complete, click Submit. a. If a signature is required, a message will appear asking if you want to review or sign the document. Click the blue Sign button and you will be taken to a page to create an electronic signature. Select the blue Create New Signature tab. b. With your curser on a computer or with your finger from a tablet/mobile device, create your signature then click the blue Save tab. You will see your signature displayed. Select the gray Done tab underneath. Once complete, you will then be on your account management page. Select Home near the top left of the screen. c. Once you have created an e-signature, you will not be required to repeat this process. You will be able to apply this e-signature anywhere an e-signature is required. 2. Repeat the above steps for the remaining forms. (You will be required to create your signature only once.) Step 4: Join a Team(s): 3. After completing the forms, you MUST join a team/group. To join a team/group, click Update next to Joined Teams, and check the box next to the correct team(s) or group(s) you will be joining. You must join the 2019 New/Transfer/Re-Admitted team if you are a NEW / TRANSFER / RE-ADMIT student. If you are a student athlete on a WC team please join the appropriate athletic team. You cannot skip step #4 as it is critical for the staff to be able to view and approve your documents. Step 5: Upload a copy of Medical Insurance Card and, if applicable, your Immunization Record and/or Insurance Disclosure: 1. From you Home page, click the Manage Documents button. 2 P a g e

3 2. Click Choose File to select your document. Clicking Choose File on a mobile device or tablet will open your device s camera, prompting you to either choose a photo from your library or take a new picture to upload. 3. Click Document Type and select from the drop down list. You also have the ability to add a comment if you would like to provide additional information. 4. When you are finished and are ready to upload your document click Upload. When uploading your Immunization Record, if you have more than one page and you are uploading them individually you must ARCHIVE each page prior to adding an additional page. a. Upload the first page b. Click the Manage Documents tab c. Click your current immunization record, the area will turn yellow. d. Click the e. Click the File Cabinet archive button. f. Click the RED Archive button. g. Click the blue Upload Document button and upload your second page, following the same instructions. 3 P a g e

4 Checking your Clearance STATUS 1. Log into your account. 2. Click or hover over Clearance Status. A box will generate as seen below. In this instance the student has not turned in Immunization Records and it is noted in the box. Three Designations: Pending your account has not been reviewed. If it has been more than 7 days please send us an , as we are not notified when new accounts populate. WChealthcenter@wilmington.edu. NOT Cleared you are missing something. Your account has been reviewed and a note has been made. Check for the note by hovering over the status bar. Cleared with Restrictions ALL students will receive this status once they have cleared the Health Center. If you are also an athlete, Athletics will provide the final Cleared once you have had your on campus physical and are fit and cleared for participation in your sport. 5 During the months of May-August and December please be aware that Student Health Services operates on a limited schedule. Please allow 7 days for information to be processed. 5 P a g e
