1 WEB TO LEAD FORM This function gives Prospects the same opportunity as Members and Guests to enroll online. Users can fill in the Web to Lead Form, submit, and receive a confirmation message. The prospect record is then created and any notification rules tied to adding a prospect are applied. For facilities that currently use a form on their website to capture prospects (and then manually enter them), there is a form in Spectrum that can be embedded into a facilities website. All the information captured in this form will be stored in the Spectrum database. To embed the form using an html iframe command, use the script at the end of this document. If you are not going to embed the form using iframe, you would utilize a different page for the form, via a link on your page. 3 To set up the form, go to Site Administration > Online Services > Member Online Setup > Web to Lead tab. On this tab you can associate the Default Salesperson, and the Default UDF screen for the lead source. You can also specify up to 2 UDF screens and 2 Member Codes that must be filled out by the prospect in the online process. You can set up UDFs and Codes that will be used on the OLS form at Site Administration > Member Management > User Defined Field Pages (and > User Defined Fields). You can set up codes at Corporate > Members > Categories / Codes. [Note: You must put in an employee as Default Salesperson. This can be set up to send an notification to the employee.] [Note: For the Default UDF for Lead Source field You will need to specify a UDF that will be filled in automatically by the form.} You can set up UDFs and Codes that will be used on the OLS form at Site Administration > Member Management > User Defined Field Pages (and > User Defined Fields). You can set up codes at Corporate > Members > Categories / Codes. In order to set up an automatic notification , see the instructions at the end. When the user has successfully saved their form, they will get a confirmation message. 3

5 To have the OLS lead form automatically generate an to the Default Salesperson selected above, follow these steps: 1. Go to Corporate > Notification Manager > Add New button 2. Type a name for the rule in the Rule Description field. 3. Select Prospect in the first drop down. 4. Select Is Created in the second drop down. 5. Select With UDF in the third. 6. Click Next. 7. In the new screen that opens, click the Select link. 5

6 8. Select the UDF that you have created to associate with the Web to Lead form. 9. Click Select, and select the value that, if selected, will cause this system rule to be activated. Then click Next. 6

7 10. In the next screen, click the drop-down and select Send an Choose where to have the notification go (It can go to an Employee Role or to a specific Employee). Also fill in the fields for Send From, Subject, Send in, and Template. 7

8 12. Click Done in this screen, and then Finish in the last screen. 13. You can also set up a rule that will trigger a welcome to be sent from your facility to the prospect. Templates can be set up in Corporate > General Setup > Templates. Select Notification Manager in the module drop-down. In the Select Type drop-down, select the event that triggers the sending of this particular . 8

9 Click on an existing template to edit it, or click the Add New button to create a new message. This template will be available to be selected in the workflow above. iframe for login and web to lead pages If you want to brand the login page, send the following HTML code to your Web master. The code can be placed where you would like to display the login screen.
