2 Ideas on HOW TO USE IT When you request a custom version of the Hereditary Cancer Quiz, you have many options on how to direct your patients to fill it out and make sure you receive the results. There are many options available to integrate the Hereditary Cancer Quiz into your practice. The tool is compatible on all devices and takes 6 minutes on average to complete. Here are a few ideas on how you can use the Hereditary Cancer Quiz in your daily practice: Embed Code Custom URL Post it to your website and ask patients to complete it before they come in for an appointment. New patients as well as survivorship patients are appropriate for this. Send your custom url/link to your patients prior to their appointment via Give the custom url/link to your patients on a printout along with the other intake materials and ask them to bring in their results Use technology in the waiting room (tablets, kiosks, etc) and ask patients to fill it out while waiting Print your custom url on the postcards available through Myriad to hand out to patients in the waiting room or to mail to patients before their appointment Link the custom url to your patient portal and request patients to complete it Embed the Hereditary Cancer Quiz on your website and direct patients to go there and complete it before coming to the appointment or while in the waiting room Create a link in your patient portal that drives patients to the embedded Hereditary Cancer Quiz on your website and ask them to complete it

3 How to Request a CUSTOM VERSION Visit and go to the For Your Practice section under Assessing Patient Risk Click on Custom Hereditary Cancer Quiz Request a custom version by filling out the form and clicking submit Form information includes: Name Address Zip Code Specialty After submitting your information, your local Account Executive will be notified of your request. If you are not contacted within 5 business days, please send an to contactus@myriad.com Your local Account Executive will contact you within 5 days to provide you with your custom Hereditary Cancer Quiz information. Your Account Executive can also help you get started using the tool. Additional customization is available. Please contact your Account Executive for more information on how to: Add or change your logo Change the colors to match your branding Change how you receive results - , print or both Update the instructions to the patient at the end Other custom changes based on customer needs

4 Security and PRIVACY We understand that patient privacy and security is very important, therefore, Myriad Genetic Laboratories has taken significant steps to make sure that the Hereditary Cancer Quiz follows all protocols to protect patient information. Receiving Hereditary Cancer Quiz Results via Secure Link When patients finish the Hereditary Cancer Quiz, they are prompted to submit their results specifically to the provider or practice that requested the custom version. Once the patient clicks submit, result notifications are sent to the verified address associated with that specific custom url/link through a secure server. 1) Once you receive the result notifications via , you will need to click on View Message. 1 2) A secure mailbox will then open. Next click on the file labeled HCQ_RESULTS to view a patient s Hereditary Cancer Quiz results. You may also save a patient s results by checking the box next to the file and clicking Save Checked Files. To help identify and easily sort your quiz results, we have included the patient name in the subject line of the . 2 Secure Mailbox
