3 OVERVIEW This form is used to notify a student s parent or guardian regarding any minor changes made to the IEP. The form documents the agreement between the district and the student s parents not to reconvene an IEP team meeting. FORM INSTRUCTIONS CREATE FORM STEP 1 SELECT A STUDENT 1. Click Student Management > Manage an Existing Student. 2. Use the Search window to find the student based on either, Last Name, First Name or Student ID. 3. Click Search. Then click the appropriate Name to select a student. STEP 2 CREATE FORM Once a student has been selected, the student s Name, ID, Birth date and Age will display. The available forms will be listed in the student tool bar located on the left side of the screen. The Program Amendment form is third from the bottom. 1. Click on the Program Amendment link. The Form Summary screen will display, listing any prior forms for the student with the most recent form at the top of the list. 2. To review an existing form, click the appropriate form/date link. If a meeting had been previously scheduled in IEPPLUS, select it from the drop down list. If not, select Create a Meeting, and then enter the Meeting Date.

4 If creating a new form, select a Letterhead to print on the form. This will default based on System/User options as defined by the district. The letterhead determines the name and address information on the first page of the form. The letterhead can be changed. If a meeting had been previously scheduled in IEPPLUS, select it from the drop down list. If not, select Create a Meeting, and then enter the Meeting Date. Meeting Type will default to IEP Team Meeting. The type will display on the Summary page that lists all Program Amendment forms for the student. Select an IEP form to attach from the drop down list. When finished with selections, click on the Create Form button. NAVIGATION/TOOL BAR After the form is created it will display to the screen. A tool bar will display on the left side of the screen, divided into the following areas: 1. Sections There is a separate link for each section of the form. Click on any link to move to that section. 2. Maintenance These options will maintain Form Information, such as Form Name, Date Ranges, Letterhead, etc. Refresh and Delete functionality is available depending on user security SunGard Public Sector Inc. 5 3. Validation These options will change the status (i.e., Locked, Ready for Review, Finalized). 4. File Attachments Click the plus icon (+) to add an attachment. Click the minus icon to delete an attachment. 5. Archives Archives are permanent PDF copies of the form. Click Finalize in the Validation section to create an Archived copy. The form should be finalized and archived only after it has been fully completed and a copy has been sent home to the student s parent(s) or guardian(s). The following options will display at the top and bottom of the form (see screenshot below). Save Information from each section should be saved before moving to another section. Return Click the Return icon to return to the Form Summary screen. Print Preview Print Preview will display the form in Adobe Acrobat. Mouse over the bottom of the form preview to see the toolbar illustrated below. Select the Print icon or click the Save icon to save a copy of the form to your computer. 6 FIELD MAPPING This section provides data source and destination information, where applicable, for all fields on the form. If source or destination data is missing or incorrect, follow the navigation indicated in this section to add or correct the data. Also included are the details of any prior setup needed, such as the Alternate Codes used for state reporting purposes. To add, edit or lookup a code, navigate to Maintenance > Manage Code Tables. Select the name of the table from the drop down list. Then click the Show Alternate Codes link at the top of the screen to see the applicable codes (see section on Code Table Setup for more information) SunGard Public Sector Inc. All Rights Reserved. 7 1 Logo, Letterhead & Report Headers IEPPLUS Illinois Transfer of Rights Form Letterhead selected when creating the form or maintained in Form Information. Letterhead will default based on settings maintained under Options > Set System Wide Options or Set My Options > Main. Letterhead requires prior setup under Maintenance > Manage Letterhead. The district Logo will print on the report, if an image file is added to the client images folder in the IEPPLUS directory (see Help > Administration > District Logo for more information). 1a Report Header 1 Maintenance > Manage Letterhead > Staff/Enclosure of Type = Report Header 1 1b Report Header 2 Maintenance > Manage Letterhead > Staff/Enclosure of Type = Report Header 2 2 Form Date Date the form is created. 3 Student Name Student > Demographics > Name: First, Middle, Last and Qualifier. 4 Date of Birth Student > Demographics > Birth Date. 5 Parent/Guardian Address & Salutation Basic Information > Contact, Type = Parent/Guardian, or Surrogate Parent where Addresses Tab = Mail If Contact > Mail attention line starts with "To the", then the attention line without "to the" will print. Otherwise, the contact salutation + the contact last name will print. 6 Meeting Date/Attendee Meeting linked to the form in Form Information will populate the Meeting Date and Attendee information. The Meeting Date and Attendees will display as active links, i.e., clicking on either link will display the meeting record where the user can change the attendees, meeting date, meeting type, etc. The attendees list will display both Name and Position, omitting parent and student. 7 Contact Method Check boxes used to indicate how the parent was contacted (ex. spoke on the phone, met in person). 8 Summary & Explanation Text box used to describe the changes and the rationale for the changes. 9 Date Changes Begin Text box used to enter the date the changes will start. 10 Form Contact Name Default to: Letterhead, Staff/Enclosure of Type = Procedural Safeguard Notice Contact Line 1. User can change the data. 11 Form Contact Title Default to: Letterhead, Staff/Enclosure of Type = Procedural Safeguard Notice Contact Line 2. User can change the data. 12 Form Contact Phone Default to: Letterhead, Staff/Enclosure of Type = Procedural Safeguard Notice Contact Line 3.
