OVERVIEW The Custom Form option allows the user to create custom forms with data merged from a student s record. Note, it does not create student data in IEPPLUS.

3 OVERVIEW The Custom Form option allows the user to create custom forms with data merged from a student s record. Note, it does not create student data in IEPPLUS. There are two steps required to implement the Custom Forms option: Clients must enable this option to be to visible from Manage Screen Configuration Clients must assign Security Permissions SCREEN CONFIGURATION TURN ON ADDITIONAL FORMS Turn on the ability to display custom forms by going to Home page in IEPPLUS and Select Maintenance > Manage Screen Configuration. Select MainPage from the Select a Form dropdown menu. Select edit on the row labeled ManageCustomForms. Changed the Visible column to Yes by checking the visible checkbox. After changing make sure to click Save. STUDENT NAVIGATION BAR Next, turn on the ability to see a custom form on the student s toolbar by selecting Maintenance > Manage Screen Configuration. Select StudentNavigationBar from the dropdown menu. Next select edit on the row called AdditionalForms. Changed the column Visible to Yes. SECURITY CONFIGURATION SECURITY CONFIGURATION SETTINGS There are two (2) security settings available. 1. One defines who can create form templates for your users. 2.

The other defines who and what staff can access these forms. NEW FORM CREATION SECURITY PERMISSIONS SETUP The ability to use a custom form is controlled by Security, Manage Group and/or User: Choose the Permission Group you wish to grant access to create custom forms through Manage Custom Forms. Find the Maintenance section in Permissions and then the line = Custom Forms. Check the appropriate check boxes indicate whether a person/group can Add a new form and/or Edit an existing form.. ABILITY TO SEE CUSTOM FORMS The ability to see custom forms on a student s toolbar is based on security and is controlled by Security, Manage Group and/or User: Choose the Permission Group you wish

5 ABILITY TO SEE CUSTOM FORMS The ability to see custom forms on a student s toolbar is based on security and is controlled by Security, Manage Group and/or User: Choose the Permission Group you wish to grant access to add and/or edit a student s custom form. Find the Additional Forms section in Permissions. Check the appropriate check box to indicate whether a person/group can View form. CREATION OF A CUSTOM FORM TEMPLATE CREATE A NEW FORM IEPPLUS, Home page: Select Maintenance > Manage Custom Forms. MAIN TAB To create a New custom form, choose the New Icon. This will display the Main screen for Custom Forms. Give the form a name. This name will be displayed within student records. Also enable the check box for Active SunGard Public Sector Inc. MEMBERSHIP TAB Next, click on the Membership button. This will display the Membership screen for Custom Forms. Check the actions that this group/user can accomplish.

Add: Enables members of the security group to create a custom form in a student s record. Edit: Enables members of the security group to change a student s custom form.

6 MEMBERSHIP TAB Next, click on the Membership button. This will display the Membership screen for Custom Forms. Check the actions that this group/user can accomplish. Descriptions of actions are listed below: View: Enables members of the security group to only view the custom form within a student s record. Add: Enables members of the security group to create a custom form in a student s record. Edit: Enables members of the security group to change a student s custom form. Delete: Enables members of the security group to permanently remove a student s custom form. Lock: Enables members of the security group to convert the student s custom form to a locked pdf file. Unlock: Enables members of the security group to only unlock a student s locked custom form. Finalize: Enables members of the security group to create an archived copy and lock the custom form. Copy: Enables members of the Security Group to create a new custom form by copying a previous custom form. Next, click on the Form Template button. This will display the Template Workbench screen for Custom Forms. This is where the body of the form is created. There are two tabs within this screen, Template Editor, and Template Preview. The Template Editor screen is divided where the top narrative box is where the form is designed and below this box is where the choice of IEPPLUS data tables exists SunGard Public Sector Inc. In the above example we chose Line 1 through Line 8 and then clicked on Save under New Letterhead.

7 ADDING INFORMATION ON A FORM TEMPLATE In the following example we are adding Letterhead to the body of the form: Under choose a table, select Letterhead and select Add Enable all check boxes where you would like to pull data from. In the above example we chose Line 1 through Line 8 and then clicked on Save under New Letterhead. o The screen will change after selecting save and will look like this: SunGard Public Sector Inc. ADDING FIELDS ON THE FORM TEMPLATE EDITOR Next, copy the contents of the grey boxes and paste within the narrative box above. In this example we copied and pasted Line 1 through Line 8.

TEMPLATE PREVIEW Select the tab at the top called Template Preview. The following screen will be displayed. o This screen only displays sample data. You will not see any district data in the Template Preview. This is allowing you to view the layout of your custom form using sample data only SunGard Public Sector Inc. All Rights Reserved. Don t forget to select Save at the top of the Custom Form along the way to save your new creation.

9 ADDING MORE FIELDS ON THE FORM Choose the tab of Template Editor to return to editting your custom form. In this example we are adding text To the Parent(s) \ Guardian of: to the body of the form and then the student s name by adding the name data field from the Demographic data table. You will follow the same process as letterhead by copying field information located in gray into the editor window. If you wish to preview your form again, select the Template Preview tab. Don t forget to select Save at the top of the Custom Form along the way to save your new creation. Data from the following tables in IEPPLUS is available for selection. USING A CUSTOM FORM WITHIN A STUDENT S RECORD Once everything has been setup, you will now see Additional Forms on your Form Toolbar inside a student s record.

CREATE A NEW FORM FOR A STUDENT Upon selecting Additional Forms, the screen will change to the following. Based on security settings you may be able to create a new form or just view existing forms. Selecting the drop down will display the custom forms available. Selecting a specific form and New will now create that form for that student. ADDING IEPPLUS DATA ON THE FORM Once the form is displayed, you will notice fields on the bottom of the report SunGard Public Sector Inc. All Rights Reserved. Page 10 of 13

12 Select student data from the drop downs. View/Modify enables the user to either enter manual text or allow modification of existing data. For Example: The selected home phone number is incorrect; the user can modify the actual phone number on the form to complete the process. (NOTE: This change DOES NOT affects IEPPLUS or eschoolplus data. Existing procedures for data correction must still be followed. ) Select Save from either the top or bottom of the form. PREVIEW REPORT WITH DATA Next select Form Preview. You will notice that all areas represented as { } in Form Editor are now filled in with data form IEPPLUS.
