3 OVERVIEW The WA Indicator 7 is a way for states to determine the percentage of preschool children with IEPs who show an improvement in the three outcome areas listed below: 1. Positive Social-Emotional Skills (Including Social Relationships) 2. Acquiring and Using Knowledge and Skills 3. Taking Appropriate Action to Meet Needs The Child Outcomes Summary Form (COSF) give providers a way to summarize student info gathered from multiple sources to address the three outcome areas listed above. The 7-point scale (1 = Not Yet, 3 = Nearly, 5 = Somewhat, and 7 = Completely ; even numbers are not labeled) determines the extent in which a student's functioning on each outcome area is age-appropriate, and if the student made progress towards that behavior. FORM INSTRUCTIONS FORM CREATION The Indicator 7 form creation process includes the following steps: Step 1: Select a Student Step 2: Select Options STEP 1 SELECT A STUDENT 1. Click Student Management > Manage an Existing Student. 2. Use the Search window to find the student based on First Name, Last Name, Student ID, Birth Date, Caseload Only, and Active Only. The user may also filter the results using Filter by Type such as Special Education, Referral, etc. 3. Click Search. Then click the appropriate Name to select a student. 4. Once a student has been selected, the student s Name, ID, birth date, current Age and Grade will display. The Forms that can be created for a student will be listed under the Forms section. Click on the Indicator 7 COSF link SunGard Public Sector Inc. All Rights Reserved. Page 3 of 27

4 The Indicator 7 COSF Summary screen will display, listing any prior forms created for the student. The most recent form will display at the top of the list. To review an existing Indicator 7 COSF form, click the appropriate link. To create a new Indicator 7 COSF form, click the New icon. If the form is unfinalized, then the summary screen will display that information as well in a red font. STEP 2 SELECT OPTIONS 1. Select an ARD to attach to the Indicator 7 COSF form from the drop-down list. 2. Enter the form s Date Range (Start Date & End Date). Note only the Start Date is mandatory. 3. Select the Service Date for the form. Note, upon creation of the form, this page will display the current system date, but the user may change it if needed. 4. Click the Create Form button to create the COSF form. Alternatively, the user may click the Cancel button to discard any changes made to the screen. Clicking Cancel returns the user back to the Indicator 7 COSF Summary screen. NAVIGATION/TOOL BAR Once the COSF form opens, it displays the Main section by default. A toolbar will display on the left side of the screen, divided into the following areas: 1. Sections There is a separate link for all four sections of the form. Click on any link to move to that section of the form: Main 1. Positive Social-Emotional Skills (Including Social Relationships) 2. Acquiring and Using Knowledge and Skills 3. 5 2. Maintenance These options will maintain Form Information, such as Form Date Range, Link Form To, and Current Placement Date. Also, Refresh and Delete functionality is available depending on user security. 3. Validation These options allow the user to validate the form elements ( Indicator 7 Validation ), and also change the status of the form (ex: Lock, Unlock, and Finalize). 4. File Attachments Click the Attach File(s) link to add an attachment. Click the Delete File(s) link to delete an attachment. 5. Archives Archives are permanent PDF copies of the form. Click Finalize in the Validation section to create an Archived copy. The form should be finalized and archived only after it has been fully completed. 6 The following options will display at the top and bottom of the form (see screenshot below). Save Information from each section should be saved before moving to another section. Return Click the Return icon to return to the Form Summary screen. Print Preview Print Preview will display the form in Adobe Acrobat. Mouse over the bottom of the form preview to see the toolbar illustrated below. Select the Print icon or click the Save icon to save a copy of the form to your computer. The plus (+) icon will enlarge the preview for ease of viewing. REVISING/AMENDING AN INDICATOR 7 COSF To revise a finalized Indicator 7 COSF form, click the Revise link illustrated in the screenshot below. This Revise link only appears for finalized active or future Indicator 7 COSFs. If the form has already expired, the user will not see the Revise link. Clicking this link will unlock the form for editing. The archive copy will update with an O to denote that it is the original version and does not include the changes. NOTE: The Revise link can be relabeled to do this, navigate to Home > Maintenance > Manage Screen Configuration > FormSummary. From here, the user can edit the Item Name, change the Display Text, or hide it: SunGard Public Sector Inc. All Rights Reserved. Page 6 of 27

7 REMOVE REVISION/AMENDMENT This functionality allows the user (based on security permissions) to remove an Revision/Amendment when the form was inadvertently amended/revised. To remove the Amendment/Revision and return the form to its previous state, click on the Remove Amendment link (Screen #1) from the Form (e.g. IEP) Summary page. Upon clicking on the link, a pop-up screen (Screen #2) will display, alerting the user that any modifications they made to the form must be manually changed back prior to the Remove Amendment process. The Remove Amendment process does not remove any changes that were made after the form was revised. Clicking the OK button will perform the amendment removal process, while clicking the Cancel button will return the user back to the Form Summary Page and the form will remain in the revised state. The Remove Amendment process will perform the following: Re-finalizes the form. Adds additional information in the Form History area of the form (see Form History section below for more details). Changes the archived form (pdf file). Please refer to "item #5 Archives" under the "Navigation/Tool Bar" section of this document for further screenshot examples. o If the form is the Original Archive and the Remove Amendment link (Screen #1) is chosen, then the O icon on the Archive is removed. o If the form is the Revised Archive and the Remove Amendment link (Screen #1) is chosen, then the R icon on the Archive is removed. 8 Screen #2 FORM HISTORY Upon removing the revision, two audit records will be inserted into the Maintenance > Form Information > Form History indicating the Remove Amendment process: 1. The first record displays the Amendment Removal process, date, time and user. 2. The second record displays the form returning to the Lock state, date, time and user. SECURITY To grant a group or user permission to the Remove Amendment link, navigate to Security > Manage Groups > Permissions, or Security > Manage Users > Permissions, then check on the Edit checkbox on the Permission > Form > Remove Amendment line item and then click the Save button. 9 SCREEN CONFIGURATION The District has the ability to change the Amendment Removal label. Navigate to Maintenance > Manage Screen Configuration > Form Summary and then click on the Edit button on the Item Name called Remove Amendment. In the Display Text column, rename it as desired, and save the changes. FORM COMPLETION There are 2 times in which this form is completed once when the student enters a program, and then again upon exit of the program. It is recommended that the form be completed for each child in a program on an annual or biannual basis. There should be a minimum of 6 months between form completions if less than 6 months, then the student will not be reported on the Indicator 7 Advanced Report. The Advanced Report is detailed in a separate document. MAIN This section consists of 4 sections: Child Information, Exit Reason/Exit Data, Summary Ratings Persons, and Family Information on Child Functioning. 1. Child Information This data is automatically populates according to the student s Demographics, and Disability record(s) and Placement record. 2. Exit Reason/Exit Data The user will select either one Exit Reason or one Exit Data when the student exits the program. The application will prevent the user from selecting more than one checkbox. **Note this section is only completed when the student exists the program. Exit Reason choose 1 of 2 checkboxes as appropriate, or Exit Data choose 1 of 2 checkboxes as appropriate 3. Persons Deciding Summary Ratings This table consists of those persons involved in deciding the summary ratings. To choose a new person, click on the Add Staff link, enter the appropriate Name & Role, and then click on the Update link to add the entry, or click on Cancel to disregard the entry. To edit an existing entry, click on the Edit link, enter the appropriate Name & Role, and then click on the Update link to save the entry. To delete an existing entry, click on the Delete link. 4. Family Information on Child Functioning This is a set of up to 4 checkboxes in which the user can indicate any information as obtained from the student s family SunGard Public Sector Inc. POSITIVE SOCIAL-EMOTIONAL SKILLS (INCLUDING SOCIAL RELATIONSHIPS) IEPPLUS: WA Indicator 7 This section consists of three sections: 1) a number scale, 2) supporting evidence for the number scale, and 3) new skills/behaviors in which the user will answer questions about the child's functioning in these 3 areas and closely related areas (as indicated by assessments and based on observations from individuals in close contact with the child): Relating with adults Relating with other children Following rules related to groups or interacting with others (if older than 18 months) 1. Number scale the user will select one textbox to indicate what extent the child shows ageappropriate functioning, across a variety of settings and situations, on this outcome. This is a number scale ranging from 1 (Not Yet) to 7 (Completely). 2. Supporting Evidence this consists of 4 textboxes and 1 checkbox. The user will describe the supporting evidence for the answer to the number scale rating given in #1 above. 3. New Skills/Behaviors the user will describe any new skills or behaviors related to positive socialemotional skills (including positive social relationships) since the last outcomes summary. **Note this should only be addressed if there was a previous rating given in #1 above. 2. ACQUIRING AND USING KNOWLEDGE AND SKILLS This section consists of three sections: 1) a number scale, 2) supporting evidence for the number scale, and 3) new skills/behaviors in which the user will answer questions about the child's functioning in these 3 areas below and closely related areas (as indicated by assessments and based on observations from individuals in close contact with the child): Thinking, reasoning, remembering, and problem solving Understanding symbols Understanding the physical and social worlds 1. Number scale the user will select one textbox to indicate what extent the child shows ageappropriate functioning, across a variety of settings and situations, on this outcome. This is a number scale ranging from 1 (Not Yet) to 7 (Completely). 2. Supporting Evidence this consists of 4 textboxes and 1 checkbox. The user will describe the supporting evidence for the answer to the number scale rating given in #1 above. 3. New Skills/Behaviors the user will describe any new skills or behaviors related to positive socialemotional skills (including positive social relationships) since the last outcomes summary. **Note this should only be addressed if there was a previous rating given in #1 above. 3. TAKING APPROPRIATE ACTION TO MEET NEEDS This section consists of three sections: 1) a number scale, 2) supporting evidence for the number scale, and 3) new skills/behaviors in which the user will answer questions about the child's functioning in these 3 areas below and closely related areas (as indicated by assessments and based on observations from individuals in close contact with the child): Taking care of basic needs (e.g., showing hunger, dressing, feeding, toileting, etc.) Contributing to own health and safety (e.g., follows rules, assists with hand washing, avoids inedible objects) (if older than 24 months) Getting from place to place (mobility) and using tools (e.g., forks, strings attached to objects) SunGard Public Sector Inc. 20 PRINTING THE FORM 1. In order to print the form, navigate to the upper-right corner of the screen, and on the toolbar will be a Print Preview drop-down as illustrated below: 2. Click on the Print Preview link and the form will open the PDF file in Adobe Reader. 3. Mouse over the bottom of the form preview to see the toolbar illustrated below. Select the Print icon or click the Save icon to save a copy of the form to your computer. The plus (+) icon will enlarge the preview for ease of viewing. FORM VALIDATION RULES Form validation is a feature that will check for missing or invalid data within a form (ie. Indicator 7) that is based on a series of specified pre-written rules. A series of validation rules will be checked within the form and will display a screen listing any errors and warnings when particular rules fail. There are two types of validation messages; Errors (identified with a red x ) and Warnings (identified as a yellow triangle). See Help\Forms\Form Validation for more information. The Validation Rules can be maintained by navigating to Maintenance > Manage Compliance Collections > Indicator 7 Validation > Collection > Main. Next, choose the following drop-down filters and checkbox to view the active Validation Rules: Name = Indicator 7 Validation Type = Form Validation Kind = Indicator 7 COSF Active = checkbox on SunGard Public Sector Inc. 22 SETUP AND CONFIGURATION SECURITY User or group level security permissions will control access and functionality based on least restrictive settings (group or user level security). In other words, a user assigned to a group (or groups) will have both user and group level permissions, and the least restrictive settings will take precedence. Since all users are assigned to the SPIPUBLIC group, it is best practice to uncheck all permissions for this group, as well as all permissions at the user level. This will ensure the group level permissions are the least restrictive. Administrators would have all permissions by default. To view or maintain security permissions for an existing user or group, navigate as follows: Security > Manage Groups (or Manage Users). Select the group(s) or user(s) and click on the Permissions tab. Scroll down to the Indicator 7 section. Clicking the permissions check boxes will control access and functionality as defined in the table below. 23 Refresh User will have active Refresh link in the form tool bar providing the ability to refresh/update a form with all the latest information. IEPPLUS: WA Indicator 7 Refresh link in form tool bar inactive. User will not have the ability to refresh a form. CODE TABLE SETUP 1. Navigate to Maintenance > Manage Code Tables. 2. Select the name of the table from the drop down list (ex: Level of Intervention ). 3. To add a code: choose New from the top menu. 4. Enter the Code in the yellow box, and then click on Update and Save the record. 5. To add or edit an Alternate Code, first click Show Alternate Codes at the top of the page. 24 Once the record is open, click the appropriate link to add a new Alternate Code or Edit an existing one. Clicking Delete will remove an existing Alternate Code. When finished with the changes, click the Save icon. GOALBOOK SETUP This form has the ability to enter text into a text box or add canned statements stored in a goal book. Upon delivery of this new form, your existing goal book will have the necessary Category, Subcategory and Level available for selection. If your goal book does not contain the necessary categories listed in the mapping document, you will need to add the categories first, and then the actual statements. Listed below is How to add the statements to a goalbook and if necessary How to add the category names so they are available in the drop downs for selection. TO ADD STATEMENTS FOR STAFF SELECTION Go to Maintenance, Manage Goal Books. Select the appropriate goal book you wish to enter selections. Select New Statement Next create a unique code number (for example ) Select Type = OSStatement Select a Category from the drop down based on information from the Field Mapping Section. Select a Subcategory and Level as appropriate, again based on the Field Mapping Section. Enter the text description and select Save. 25 TO ADD CATEGORIES, SUBCATEGORIES AND LEVELS IN YOUR GOAL BOOK: Go to Maintenance, Manage Goal Book Codes. Select the appropriate goal book you wish to enter these codes. Select New at the top of the page. Category: Enter the Category name in the text box (i.e. Positive Skills) and select update. Repeat as needed. Subcategories: Click on the Category Name (i.e. Positive Skills) you wish to add a subcategory to. Next select New from the top of the page. Enter the Subcategory and update to save. 26 Level: To add a Level, select the Category name, Subcategory name then New and add the required code. It works the same way as outlined above. To move around this section, use the breadcrumb trails found at the top of the page. REVISE/AMEND REMOVE AMENDMENT WARNING MESSAGES A new option has been added that allows administrators to enable/disable warning messages for the Revise/Amend & Remove Amendment actions. To do this, the administrator would navigate to Options > Set Systemwide Options > Form and then enable/disable either or both of the options. By default, both warning messages are turned on (both checkboxes are unchecked). To enable the Revise/Amend warning message, the administrator should leave the 1 st checkbox unchecked. When this option is enabled, the user will see the following pop-up message when the Revise/Amend link on the Form Summary page is clicked: SunGard Public Sector Inc. All Rights Reserved.
