Other: Alt Code = Other Selection of the Other check box enables the text box below the drop-down. The drop-down displays the Meeting Reasons from the Meeting Reason code table that do not match A-D above. The data entered into the text box is saved to student s Meeting record Notes tab. Both the English & Spanish versions have been updated to include new text changes. 4/12/2012: Student Letter: Based on Age 13.0 as of day of meeting AND student s address record = Mail 9/8/2011: Addition of Choose Staff in Please Return this form To: The form will pull in letterhead, Invitation data first if it exists. If X Choose Staff is selected, these fields will be overwritten with the new staff data.
