4 If you don t have an iclickcare account, choose Create New Account 4

5 On the New User Signup screen, tap the First Name box and use the on-screen keyboard to enter your information. Then tap to advance for the next information boxes. 5 5

8 To login to an already created iclickcare account: 1. Type your user name (login address) and password. 2. Tap Login. 8

9 If you are assigned as a delegate, you can: - Tap the name of the provider who assigned you. - Tap your name if you are not acting as a delegate. 9

10 You can login and take pictures or video clips even if you are not connected to the internet. 9 10

11 If there are no patient visits, tap the ACTION button in the lower left of the screen. This will give you choices, one being to Add New Visit. 11

13 Tap to create a new patient record for your visit. 13

14 Tap the button to type in patient demographics using the on-screen keyboard

15 Fill-in all of your patient s information. Then tap Done 15

16 Complete this screen including: - Patient s History and Physical...then - Clinical category from the list of ICD s. - Coverage group indicating who has the privilege of seeing this patient visit. Then tap Done 15 16

17 09/27/2009 Open the case. Tap the Camera button to take the picture(s)...or video clip(s) (15 seconds or less). 17

18 Tap photo or video. Then tap the button in the center to take the picture or video clip

19 Choose RETAKE or USE PHOTO. The picture or video clip is then stored securely. 19

20 In the iclickcare Camera Roll, you can also add a secure note to your photo or video

21 Tap to type your note using the onscreen keyboard or tap the Microphone button to TALK using Siri (iphone 4S and above). Then tap Done. 21

22 Tap the ACTION button to: - Add your note as a comment for a visit. - Edit the note you added to your photo

24 From either the List, the Visit, or the iclickcare Camera Roll, pictures or video clips can be taken, stored, 24

25 Of course, you must be logged-in to iclickcare to take a picture or video clip on the private camera roll. 25

26 Tap the ACTION button in the lower left of the screen. Then tap Add to Visit to add your photo or video clip to a visit. 26

27 The picture is added to the Visit. You may tap to enlarge it

28 In either portrait or landscape mode, tap again to remove banners. Then pinch to reduce, or spread to enlarge, the picture further. 28

29 The iclickcare Camera Roll is private. These pictures are NOT in your iphone roll. 29

30 Tap to take a new picture or video clip for this visit; for another visit for the same patient; or for a new patient. 30

31 Tap the ACTION button on the Visit to Request Collaboration from your colleague

32 You may direct your question to a specific consultant. Other providers for this patient may be invited to collaborate as well, and they are auto-notified. 32

39 On any page, tapping the ACTION BUTTON gives you related choices. 39

40 To create another visit: - Tap the ACTION button - Tap Add New Visit - Fill-in the updated Hx and PE - Select a visit date. 40

41 Tap on the patient name to create a visit for a different patient OR add a new patient. 41

42 To create a new patient record, tap the button. 42

43 To add a visit for an EXISTING PATIENT, tap the desired name from your list of patients. 43

44 Tap the ACTION button to Invite Patient and have a two-way communication with your patient or a family member

45 When a patient or family member is invited to join this visit, a secret password is generated, and the invitation is noted in the visit comments

47 Tap the ACTION button to Compare Visits and check the improvement or worsening of a condition, or study the clinical changes over time

48 The marquee image from one visit is compared to the marquee image from the patient s subsequent visit. 48

51 Tapping a picture on either the List or the Archive view will take you to that Visit

52 If Favorites have been designated, they can be viewed by tapping. 52

53 Alternatively, you can Search with an automatic flip back to the List View, or go from the ACTION button to the Camera Roll

55 To set a secure passcode for a quick login if iclickcare has been inactive for more than 5 minutes: - Tap the ACTION button from the List. - Tap Settings. 55

56 Slide the Passcode to ON. Enter a 4-digit passcode and tap OK. Then tap Done 49 56

57 Refresh often to see data entry changes. To log out of iclickcare, press the Home key below
