4 Tonight s Agenda (Action Item No. 6) Approval of Independent Consultant Agreement No. 327 with BPI Inspection Services for the Jefferson Elementary School ORG Building DSA Inspection Services On May 6, 2014 the Board approved the Construction Schedule for all Ten ORG Projects Proposal from BPI Provides DSA-Required Project Inspection Services Not-To-Exceed Amount of $204,000 is Included in the Approved Budget Measure S and State ORG Funds Superintendent s Facility Advisory Committee - Supported GUSD STRATEGIC PLAN: Students will learn in safe, well-maintained facilities. 4

5 Tonight s Agenda (Action Item No. 7) Approval of Independent Consultant Agreement Nos. 324, 325, 326, and 328 with Sandy Pringle Associates for the Lincoln, Fremont, La Crescenta, and Muir Elementary School ORG Buildings DSA Inspection Services On May 6, 2014, the Board Approved the Construction Schedule for all Ten ORG Projects Proposal from Sandy Pringle Associates Provides DSA-Required Inspection Services Not-to-Exceed Amount of $204,000 per School, totaling $816,000 for all four (4) Schools, is Included in their Approved Budgets Measure S and State ORG Funds Superintendent s Facility Advisory Committee - Supported GUSD STRATEGIC PLAN: Students will learn in safe, well-maintained facilities. 5

6 Tonight s Agenda (Action Item No. 8) Approval of Project Authorization No. 22 with tbp Architecture for Installation of Portable Buildings at Cloud Pre-School On February 2, 2016, the Board Approved a Budget Allocation for the Installation of Portable Classrooms at Cloud Pre-School Contractual Agreement, Budget, and Timelines were Established with tbp Architects for this Project with Architectural Fees Totaling $82,950 District is Exercising a Credit of $1,659 for Previously Completed Schematic Design Work and $43,786 for Work Previously Billed, Resulting in Total Remaining Fees of $37,505 Developer Fee Funds Superintendent's Facility Advisory Committee - Supported GUSD STRATEGIC PLAN: Students will learn in safe, well-maintained facilities. 6

7 Tonight s Agenda (Consent Item No. 8) Approval of the Notice of Completion with Borrego Solar Systems, Inc. for Installation of Photovoltaic Systems at the New College View School On February 3, 2015 the Board Approved the Award of Contract to Borrego Solar Systems, Inc., for Installation of Photovoltaic Systems at College View School Project had No Change Orders and was Completed in a Satisfactory Manner as of February 10, 2016, for a Total Cost of $850, Measure S New College View School Funds Superintendent s Facility Advisory Committee - Supported GUSD STRATEGIC PLAN: Students will learn in safe, well-maintained facilities. 7

8 Tonight s Agenda (Consent Item No. 9) Approval of the Notice of Completion with Chalmers Construction Services, Inc. for Bid No /15 for the Network Operations Center (NOC), Administration Building B Basement Remodeling On September 2, 2014, the Board Approved an Initial Agreement with Chalmers Construction Services, Inc. for the Remodeling of the Network Operations Center for $1,780, There Were Three (3) Change Orders on the Project Totaling $134, and Representing a 7.6% Cumulative Change to the Original Contract The Project was Completed in a Satisfactory Manner as of February 22, 2016, for a Final Cost of $1,914, Measure S Technology Funds Superintendent s Facility Advisory Committee - Supported GUSD STRATEGIC PLAN: Students will learn in safe, well-maintained facilities. 8

9 Tonight s Agenda (Consent Item No. 10) Approval of the Notice of Completion with Convergint Technologies for Installation of Security Surveillance Systems at Franklin Elementary School On August 18, 2015, the Board Approved the Award of Contract to Convergint Technologies for Installation of All Cameras and System Components at Franklin Elementary The Project Had No Change Orders and Was Completed in a Satisfactory Manner as of February 25, 2016, for a Total Cost of $45, Measure S Safety and Security Funds Superintendent s Facility Advisory Committee - Supported GUSD STRATEGIC PLAN: Students will learn in safe, well-maintained facilities. 9
