1 NEW Student-Athlete Athletic Training Room Registration The University of Washington Athletic Training Room has a new student-athlete health record system called Presagia. This system allows for student-athletes to sign up & sign in from anywhere an internet connection is available to update important contact information, health insurance information, fill out physical forms, consent forms, and view exercise/rehabilitation programs created by your athletic trainer. The instructions provided below are meant to guide you through this process. If you have problems creating an account contact Michael Dillon at mldillon@uw.edu. 1. Log on to on a desktop or laptop computer (No phones/tablets) 2. Click on Athlete Sign Up

2 3. Enter your a. Legal First Name b. Legal Last Name c. Date of Birth must be your UW . e. Verification phrase - GoDawgs (REMEMBER this!!!) f. Click Next

3 4. You will receive an to address with your Activation Code. Log in to address and copy your Activation Code. If you have your mail forwarded, check that inbox, and check the spam folder if you can t find it easily. Paste your Activation Code into the field below. Create a password (remember your password). Enter your cell phone number. Choose and answer the 3 challenge questions. Click Submit. 5. Congratulations! You now have an active Athletic Training Room profile. You may log in and complete your profile. It is important to thoroughly complete your profile! Log in using your at

4 6. Once you have logged in please click on the edit pencil to enter/update your information. 7. Please enter your full & legal name, date of birth, gender, student ID# as your ath. #, cell phone number, address. It is very important that you use address. Do not use gmail, hotmail, etc. Click Save when you are finished.

5 8. Click on Add Address. Select address type from the drop down menu. Be sure to enter a Permanent/Parents address (if your parents are divorced you may enter both via the drop down menu) as well as a Local/Campus address. Type in parents name in description text box. Click Save after you are finished entering addresses.

6 9. From the Athlete Summary page move your cursor over the edit pencil here and select.

7 8a. Enter important medical allergies & medical conditions here. Enter address and phone number for an emergency contact here. Click Save when you are finished. 8b. Click on the next to Sport identity

8 Select your sport from the drop down menu here. Click Save. 10. From the Athlete summary page select the next to the Insurance Policy field here

9 9a. Select Insurance Policy from the drop down here. If you do not see your insurance policy name please select Other. If you are uninsured please choose Uninsured. Enter the insurance company details here. Fill in all fields below as accurately as possible. Click Save when finished. IF you have multiple insurances (medical & dental & vision) please follow the instructions above beginning with clicking the next to Insurance Policy 11. Please or text your athletic trainer a clear photograph of the front & back of your health insurance card. If your dental insurance is different than your medical insurance please include clear photographs of that card as well. You are now finished with your athlete registration process.
