3 LEARNING OBJECTIVES This tutorial will help you learn how to: Request account access Login to MyChart Understand the Proxy access process Look up benefits, claims, authorizations, etc. Understand how to send secure messages to: Change your PCP Update an address Determine pharmacy usage by accessing Rx Manager Print Insurance ID Cards

4 REQUEST ACCOUNT ACCESS If you do not have a MyChart account, you can request an access code. Complete the form and select the Submit button at the bottom.

5 RECEIPT OF ACTIVATION CODE If you received your activation code, select the button shown here to enter the code. Fill out the form and select the Next button at the bottom.

6 LOGIN TO MYCHART Enter your username and password and click Sign In

7 PROXY ACCESS Request proxy for: Children age Other Adults Print, sign and mail paperwork to Meriter Proxy Access is requested for Dependent child or adult Form is mailed to member for signature Form is signed and returned to Meriter HIM department HIM determines through verification process if should approve or needs additional information. Member is contacted that Proxy access has been given/rejected by letter

8 PROXY ACCESS Once you receive verification that proxy access is approved, you will see a new tab(s) on the right side of the home page with the dependent s name.

10 FINDING YOUR COVERAGE INFORMATION Select the My Plan link below to open the menu of options.

11 FINDING YOUR COVERAGE INFORMATION Insurance Summary Coverage Details Online Premium Payment View all of your plan details including your plan SBC and other benefit information. Lists all of your coverage information. Make a premium payment online for individual or direct billed members.
