3 1. Working with Reevaluation/DEC7 Form The Reevaluation/DEC7 form is used to specify the reevaluation details for a Special Ed child. It is a main form that is supported by the following related forms: Invitation to Conference Student Invitation to Conference (14-18) Student Invitation to Conference (18+) DEC2 Parent Consent for Evaluation/Reevaluation Prior Written Notice/DEC5 The available statuses for the form are: Draft Closed/Verified Getting Started with Reevaluation/DEC7 Form 1.1. Adding the form Change Description: The Reevaluation/DEC7 form is updated. This is available only in the web format. Adding Committee Members feature is removed from the form. To add a form, follow these steps: 1. Select a child s record from the Recent Child List or the results screen of a Child Search. 2. Click EC Forms. The EC Forms List screen is displayed. 3. Click New. The EC Form Detail screen is displayed. The form creation date, case manager s name, and status are displayed. DEC7 - Getting Started Guide

4 Field marked in red is mandatory. 4. Select Reevaluation/DEC7 as the Form Name. 5. Click Save. The EC Forms List screen is displayed. 6. From the EC Forms List, select Show In Progress. The EC Forms are listed. 7. Click the added form link. 8. Click View Form. The Errors And Warnings screen lists the Compliance Warnings applicable for the form. 9. Click View Form. The form is displayed in the web format with the following sections. IEP Team Participation Review of Existing Evaluation Determination of Needed Data Comments 10. Enter details in all the sections. 11. To confirm that you have entered all required information, click Validate. The Errors And Warnings window displays the form validation errors. 12. If errors are listed, correct the errors. You cannot Close/Verify a form, if there is any error. 13. Click Close. 14. To close/verify the form, change the Status to Closed/Verified from the IEP Team Participation screen. A message is displayed. 15. Click Save. The form is saved with the Closed/Verified status Validating the form You can validate the form to verify that you have entered all required information correctly. On validation, the Errors and Warnings window displays the list of errors (if any). You cannot change a form s status to Closed/Verified unless all the errors are resolved. -2-

5 Change Description: There is no change in the existing functionality. Only the errors are changed according to the current validations. To validate the form, follow these steps: 1. From the EC Forms List screen, click the Form you want to validate. The EC Form Detail screen is displayed. 2. Click View Form. The EC form is displayed with a list of the various form sections. 3. Click Validate. A window with form validation errors is displayed. 4. If errors are listed, correct the errors. 5. Click Close. The form is validated Compliance Warnings The compliance warnings for the Reevaluation/DEC7 form are: Compliance Warnings Starts Stops Parents must be invited to a meeting to consider reevaluation. -- When the Invitation to Conference is Closed/Verified. The IEP must be reviewed, amended, or redeveloped following Reevaluation. When a DEC7 is Closed/Verified and No is selected for any option in the Additional Data Needed section. When a new IEP/DEC4 is Closed 1.4. Previewing the form You can view a report of the current saved data entered in the Reevaluation/DEC7 form. The report is displayed in the PDF format. To preview the form, follow these steps: 1. From the EC Forms List screen, click the Form you want to view. The EC Form Detail screen is displayed. 2. Click View Form. The Errors And Warnings screen displays the Compliance Warnings. 3. Click View Form. The form is displayed with a list of the various sections. 4. Click Preview. The EC form is displayed as a PDF report. -3-

6 1.5. Deleting the form You can delete a Reevaluation/DEC7 form if the form's status is Draft. To delete the form, follow these steps: 1. From the EC Forms List screen, click the form you want to delete. The EC Form Detail screen is displayed. 2. Click Delete. A message is displayed. 3. Click OK. The form is deleted. A main form cannot be deleted if it is Closed/Verified or if related forms are attached to that main form. -4-

Managing Reevaluation/DEC7 Form The Reevaluation/DEC7 Form comprises the following screens: IEP Team Participation Review of Existing Evaluation Determination of Needed Data Comments Each section is described in detail IEP Team Participation This screen enables the addition of IEP Team Participants for the reevaluation process. Getting Started with the IEP Team Participation Screen Form Status Draft Accessed By Case Manager Adding IEP Team Participation Details To add details in the IEP Team Participation section, follow these steps: 1. From the Reevaluation/DEC7 sections, select IEP Team Participation. The screen is displayed. Fields marked in red are mandatory. 2. Select the Status. By default, Draft is selected. -5-

8 3. The Current Primary Eligibility Category(s) is displayed based on the Primary Disability selected on Special Ed screen. 4. Select the Secondary Eligibility Category(s): (if applicable). You can select multiple options from the Secondary Eligibility Category(s) list. Use the Shift key to select consecutive items and the Ctrl key to select scattered items. You can deselect options from the Secondary Eligibility Category(s) list. Use the Ctrl key to unselect highlighted items. 5. Enter the date in The following members of the IEP Team participated in the revaluation process on. 6. Enter the Name. 7. Select the Position. If you select Other, describe the participant's position in Other. 8. Enter the Date of Participation. 9. Click Add. 10. To include additional team member follow steps 6 through Click Next. The Review of Existing Evaluation screen is displayed. Editing Participants To edit details of the Participants, follow these steps: 1. From the Reevaluation/DEC7 form sections, select the IEP Team Participation section. 2. Click the team member's Name, whose details you want to edit. The corresponding details are displayed. 3. Edit the details as required. 4. Click Update. The Participant details are edited. To cancel the editing, click Clear prior to updating. Deleting Participants To delete the Participants, follow these steps: 1. From the Reevaluation/DEC7 form sections, select the IEP Team Participation section. 2. Click Delete under the Action field for the appropriate Name. A message is displayed. 3. Click OK. The Participant is deleted. -6-

10 2.2. Review of Existing Evaluation This screen enables the addition of information from existing evaluation data. Getting Started with the Review of Existing Evaluation Screen Form Status Draft Accessed By Case Manager Adding Review of Existing Evaluation To add details in the Review of Existing Evaluation section, follow these steps: 1. From the Reevaluation/DEC7 sections, select Review of Existing Evaluation. The screen is displayed. 2. Select the Review of Existing Evaluation Data from the following options; enter description. Attendance Past and current grades -8-

11 Work samples State and district-wide assessment data Relevant medical/health information Discipline reports IEP progress Other 3. Enter description in Summary of previous assessment(s) (If attaching the most current summary of evaluation/eligibility worksheet, results must be discussed below):. 4. Enter description in Summary of evaluations and information provided by the parent(s):. 5. Enter description in Summary of classroom based assessments and observation:. 6. Enter description in Summary of observation by teachers and service providers:. 7. Click Next. The Determination of Needed Data screen is displayed Determination of Needed Data This screen enables to add whether additional data needs to be collected to make specific decisions concerning the student's current educational plan. Getting Started with the Determination of Needed Data Screen Form Status Draft Accessed By Case Manager Adding Determination of Needed Data To add details in the Determination of Needed Data section, follow these steps: 1. From the Reevaluation/DEC7 sections, select Determination of Needed Data. The screen is displayed. -9-

Select Yes or No for the following options in Is additional data needed to determine: A) Continued eligibility for special education and related services: B) Present level of academic achievement and developmental needs? C) Whether any additions or modifications to special education and/or related services are needed to meet measurable annual goals and participation in the general curriculum? 3. If you select Yes for any of the questions in step 2, select one or both of the following options; enter description. Collection of the following data without formal assessment (Complete Eligibility Worksheet(s), Complete Eligibility Determination, Address IEP, and Complete Prior Written Notice.) Collection of the following data through formal assessment: (Obtain Parental Permission, Complete Eligibility Worksheet(s), Complete Eligibility Determination, Address IEP, and Complete Prior Written Notice.) 4. If you select No for all of the questions in step 2, specify the reason in If no additional data or assessment is needed, explain why: (Complete Eligibility Determination, Address IEP, and Complete Prior Written Notice.) 5. Click Next. The Comments screen is displayed. -10-

13 2.4. Comments This screen comprises additional comments that need to appear on the report. Form Status Draft Accessed By Case Manager Getting Started with the Comments Screen Adding Comments To include comments, follow these steps: 1. From the Reevaluation/DEC7 sections, select Comments. The screen is displayed. 2. Enter description in Additional Comments. 3. Click Save. -11-
