(please keep this day free to allow for Sound Checks and Rehersal):. We will be in contact informing you if you have been successfull and which heat you have been allocated. We will do our best to allocate you to the heat you desire however space is limited. At the end of each heat it will be announced who has made it through to the final. Please be aware: All acts need to come fully prepared to perform. There will be no changing facilities available. 2

3 All contestants under the age of 18 must be accompanied by an adult and complete the consent form at the end of this application otherwise their entry WILL NOT be accepted. All applications must be received by the 21st of April Midday at the latest. They can either be returned by post to: Grove House Grove Park Weston-super-Mare BS23 2QJ Or drop it into the Blakehay Theatre on Wadham Street. You can also your form to Terms and Conditions: No more than 10 people in an act. All contestants under the age of 18 must have a parent or guardian sign the attached form, giving them consent to perform; otherwise their entry will NOT be accepted. We cannot guarantee that you will be allocated the heat you have selected, although we will do our very best to accommodate for all. Photographs and videos will be taken throughout the heats and final and used in press releases to the local media. Please sign below confirming you are agreeing to this.
