1 Introduction Process andtrigger Perform this procedure only during the Benefits Open period. This allows you to use the Open application for Benefits enrollment. Prerequisites None Menu Path Use the following menu path to begin this transaction: Employee Self-Service Benefits and Pay Open. Transaction Code None Tips and Tricks At times you may need to use your scroll bar to view additional information. Check and update your dependent and physician information prior to enrolling in Benefits plans. You must complete all sections and save the information at the end of the process in order for the changes to occur. Reports None 1/30
2 Procedure 1. Start the transaction using the above menu path or transaction code. 2. Click The Show General LinksView link to review your current physician and dependents' information. 2/30
3 3. Click the View Dependents and Physicians link to edit your Benefit plan. Family Members, Dependents and Physician Information 4. Click the Close button to exit. Review the displayed information. The following steps shows how to make updates to your information. 3/30
4 Selection for Open 5. Click the Exit button to exit the enrollment form. Benefits and Pay 6. Click the Benefits Medical Information link to update your physician information. Your own physician information can be updated through the Benefits Medical Information link. 4/30
5 Benefits Medical Information 7. Click the New Physician Information button to add a new physician. Benefits Medical Information 8. As required, complete/review the following fields: Field R/O/C Description 5/30
6 Field R/O/C Physician First Name Required Description The doctor s first name. Example: Paul Physician Last Name Required The doctor s last name. Example: Robinson PCP ID# Required The physician s identification number. Example: Click the Review button to review the information for accuracy. Benefits Medical Information 10. Click the Save button to save the changes. 6/30
7 Benefits Medical Information 11. Click the Personal Information tab to update your family members/dependents' information. Personal Information 12. Click the Family Member/Dependents link to access a family member's Benefit information. 7/30
8 Family Member/Dependents 13. Click the Edit button to edit your current dependent's information. Please note that you can only edit your dependent's physician information during the Open period. Family Member/Dependents 14. As required, complete/review the following fields: 8/30

10 Family Member/Dependents 16. Click the Save button to save the changes. Personal Information 17. Click the Go to Family Member/Dependents Overview link to make some additional edits. 10/30
11 Family Member/Dependents 18. Click the New Child button to add a new dependent to the plan. The subsequent steps shows how to enter a new dependent, in this case a new child. Family Member/Dependents 19. As required, complete/review the following fields: 11/30
12 Field R/O/C First Name Required Description First name of the person. Example: Tom 20. Click the Scroll-down button to view more of the screen. All fields with the icon are required fields and must be completed. Family Member/Dependents 21. As required, complete/review the following fields: Field R/O/C Physician First Name Optional Description The doctor s first name. Example: Amy Physician Last Name Optional The doctor s last name. Example: Wallace PCP ID# Optional The physician s identification number. Example: /30
13 Family Member/Dependents 22. Click the Review button to review the information for accuracy. Family Member/Dependents 23. Click the Save button to save the changes. 13/30
14 Personal Information 24. Click the Go to Family Member/Dependents Overview link for an updated overview. Family Member/Dependents 25. Click the Exit button to return to the Personal Information screen. 26. Click the Benefits and Pay tab to access the Benefits and Pay screen. 14/30
15 Benefits and Pay 27. Click the Open link to access the enrollment application. 28. Select the Dental Blue Freedom Pre-tax radio button. You select the appropriate radio button to select the plan in which you would like to enroll, or 15/30
16 for which you would need to change the enrollment options. The following steps show you how to edit an existing plan. 29. Click the Edit Plan button to edit the selected existing dental plan. 30. Click the Selection button in the row to choose that plan option. 31. Click the Selection button in the row to choose the coverage option. 32. Click the Select Dependents button to proceed to the next screen. If you don't have dependents or if you have chosen a plan coverage that doesn't cover dependents, you still need to click on the Select Dependents button to move to the next step. 16/30
17 33. Select the Select check box to select a dependent(s) for the plan. 34. Click the Add Plan to Selection button to continue. The process is not complete until you complete the Review and Save steps. 17/30
18 35. Select the Network Blue NE-Pretax radio button 36. Click the Edit Plan button to select that medical plan. to edit the selected plan. 37. Click the Selection button to select the appropriate medical plan. Choose the plan and coverage option by selecting the appropriate row. 38. Click the Select Dependents button to proceed. 18/30
19 39. Select the Select check box to select a dependent(s) for the plan. 40. Click the Add Plan to Selection button to continue. The following steps show you how to add a new plan to your selection. 19/30
20 41. Select the Enroll radio button 42. Click the Add Plan button for FSA Dependent to enroll in the plan. to add the plan to your existing enrollments. 43. As required, complete/review the following fields: Field R/O/C Contribution Amount Required Description The dollar amount that is being contributed to the benefit plan. Example: Click the Add Plan to Selection button to continue. The following steps show you how to remove a plan or un-enroll from a plan. 20/30
21 45. Select the appropriate radio button 46. Click the Remove Plan button to select the plan you which to remove. to un-enroll from the selected plan. The process is not complete until you complete the Review and Save steps. The following steps show you how to enroll in a Benefit plan. 47. Select the Enroll radio button for Pers Fam Acc to enroll in the Personal and Family Accident insurance plan. 48. Click the Add Plan button to add the plan to your enrollments. 21/30
22 49. Click the Selection button to select the desired plan option. 50. Click the Add Plan to Selection button to continue. 22/30
23 51. Click the Review button to review your changes for accuracy. 52. Click the Save button to save the changes. 23/30
24 53. Click the Print New Plan Selections link to review or print your confirmation form/statement. You can view, print, or download your Benefits Confirmation form in PDF format. 24/30
25 Confirmation Statement 54. Click the Close button to close the screen and return to the screen. 25/30
26 55. Click the Go to Benefits Participation Overview link to review your selection. 56. Select the appropriate plan's radio button to view that plan's details. 57. Click the Show Participation Details button to display the selected plan's details. 26/30
27 Benefits Participation 58. Click the Back button to return to the previous screen. 59. Click the appropriate radio button to select another plan. 60. Click the Show Participation Details button to display the selected plan's details. 27/30
28 Benefits Participation 61. Click the Back button to return to the previous screen. You can also print a selected confirmation form from this screen by clicking the Print Confirmation Form : All Plans link. 28/30
29 62. Click the Exit button to return to the Employee Self-Service screen. 63. The transaction is complete. 29/30
30 Results and Next Steps You have successfully enrolled in Benefits during the Open period and learned how to complete related tasks, such as adding dependents, changing plans, and printing Confirmation Statements. 30/30
