3 Act 1. You get an with the TestFlight aviator icon inviting you to be a beta tester. (TestFlight is the Apple software for testing apps before they go to the App Store.) Open the only on your iphone and tap the green Accept button. 3

4 Act 2. After tapping the green Accept button, the TestFlight SIGN UP screen appears. Under Join with a new account enter your First Name, Last Name, Address and confirmation, a Password (of your choice) and a Password confirmation. Please note your TestFlight login credentials for future reference. 4

5 Act 3. After signing up for a TestFlight account, the TestFlight app downloads to your iphone and a screen opens notifying you that You are now a tester for the... team. Tap the green VIEW APPS button. The blue and white TestFlight app opens, showing no builds available. Close the TestFlight app on your iphone (by pressing the round home button below the iphone screen). 5

6 Act 4. After the TestFlight app downloads to your iphone, you get an within two business days about the availability of the newest build of the SmartPHR app. Open this on your iphone. Tap the blue and white TestFlight app on your iphone. An APPS screen opens. Tap the green INSTALL button beside the newest app in the list. The blue and green SmartPHR app downloads to your iphone. 6

7 Act 5. After the blue and green SmartPHR app downloads to your iphone, tap the app and select the blue SIGN UP button. Complete required SIGN UP entries, noting your SmartPHR User Name and Password in writing and storing the note in a safe, convenient place for future reference. 7

8 Act 6. After you complete SmartPHR SIGN UP, a Success message appears repeating your SmartPHR User Name and reminding you to write it down and store it in a safe, convenient place for future reference. Tap the green OK button and the LOGIN screen appears. Enter your User Name and Password and tap the green LOGIN button. The MANAGE MY HEALTH home screen appears. 8

9 Act 7. After logging in to the app and landing on the MANAGE MY HEALTH home screen for the first time, tap MY CLINICAL SUMMARY, and select and complete entries in each category from Background to Drugs & Interventions. Use the green SAVE button at the bottom of each screen to save your entries to your mobile phone. Return to MANAGE MY HEALTH by tapping the HOME icon at the top left of the screen. Repeat this step as often as needed to keep your information current. 9

10 Act 8. After entering current information in MY CLINICAL SUMMARY and returning to MANAGE MY HEALTH, tap MY HEALTH GOALS. From the list of goals on MY HEALTH GOALS, select up to three and tap the green SAVE button. Green checks will appear beside your selected goals. 10

11 Act 9. After selecting up to three goals in MY HEALTH GOALS and returning to MANAGE MY HEALTH, tap MY PROVIDERS' INSTRUCTIONS. MY PROVIDERS INSTRUCTIONS opens, showing your selected goals. Tap one of your selected goals. DRUGS & INTERVENTIONS opens, showing the name of the goal you just tapped above the entries you made in MY CLINICAL SUMMARY>DRUGS & INTERVENTIONS. Tap a drug or intervention, to relate it to your selected goal and a green check will appear. Tap the green SAVE button. You can relate up to two items from DRUGS & INTERVENTIONS to each of your selected goals. 11

12 Act 10. After selecting up to three goals in MY HEALTH GOALS and relating each goal to as many as two DRUGS & INTERVENTION items, on MANAGE MY HEALTH, tap MY PROGRESS. You land on an empty TIMELINE for one of your selected goals. Tap a white box at the bottom of the screen to see a different TIMELINE. 12

13 Act 11. At 6pm local time each day, you will be prompted to update TIMELINES. Tap the app and MY PROGRESS, and, on each TIMELINE, tap YES (crea7ng pink and blue dots) if today you have completely followed instruc7ons for a goal- associated drug or lifestyle interven7on. Your YES entries are automatically saved. Below the TIMELINE graph, enter goal ra7ngs or numbers. If your goal entry is correct, tap SAVE, to preserve your entries on your mobile phone and then follow Act 12 instruc7ons to backup your entries to our remote HIPAA- compliant cloud servers. 13

14 Act 12. At least once a day, back up your data to our remote HIPAA-compliant cloud servers. If your phone is lost or stolen, you can always download and log into the SmartPHR app from a replacement phone and access all your past entries. When your phone is receiving a strong mobile signal, tap the menu icon (3 horizontal bars) on MANAGE MY HEALTH and BACKUP DATA. Tap OK on the SUCCESS message confirming your backup. 14

15 Act 13. Enter in the browser on your phone or computer. Select Feedback to send us an about each experience you have with the app, including bugs you find, usability issues you encounter, and features you want in future versions of the app. 15
