The vendor must include and complete all parts of the cost proposal in a clear and accurate manner. Leflore County School District reserves the right to request additional information or clarification of a Vendor s proposal. The Vendor s cooperation during the evaluation process in providing Leflore County School District staff with adequate responses to requests for clarification will be considered a factor in the evaluation of the Vendor s overall responsiveness. Incomplete proposals will not be considered. 2. Purchases of services and materials are dependent upon E-rate funding and district needs. Proposals must be valid for ( a Five year contract) E-rate funding year and must include the statement contingent upon Erate Funding, School Board approval and The district is not under any obligation to proceed with the project until a Purchase Order is issued by the district. 3. Complete contact information is required with proposals to include vendor s E-rate SPIN (Service Provider Identification Number). 4. The Vendor must provide at least three (3) references consisting of Customer accounts that have received similar services. Required information includes name, address, address, and telephone number of appropriate contact person. 5. The Vendor must propose contracts/services which are entirely Erate eligible, or the ineligible components must be cost allocated to determine the percent of eligibility. 6. Mandatory vendor walk- through on January 8, 2018 at 9:00 a.m.

2 Wireless proposals: Leflore County School District is seeking proposals for a wireless solution that will provide robust WLAN functionality, including voice and video optimization, high reliability, and strong security. The solution must be affordable to purchase and operate in a distributed environment and configured and managed centrally. The proposed wireless solution must NOT require a dedicated controller, but the Access Points must distribute controller functionality- including services like authentication and configuration, firmware management, and other management functions to all APs on the network. Access Points must be dual radio with at least 300 Mbps per radio and integrated antennas. Vendor Requirements: Option for Discount Billing must be provided Certifications in Cat 3, 5, 6 Cabling, RCDD, Fiber Cabling; Microsoft Certified Professionals; Network +; Security +; Cisco Certified Design Associate, Cisco Certified Network Associate, Cisco Express Foundation Design Specialist Must be a registered contractor in the state of Mississippi to be considered for the cabling portions of the project. Siemon cabling materials certified and able to offer warranty on Siemon products. Other brands can be quoted. Selection Criteria: 30% - Total cost of eligible items 25% - Vendor s experience with the district 20% - Vendor who offers a Turn Key Solution 15% - Proximity of Vendor or vendor technicians to the district 10% - Vendor s Erate experience (provide supporting information) All equipment must be branded as listed or equivalent Below is a detailed list of all components to be quoted:
