1 Create a new My PHLY account: From the PHLY.com home page, select one of the following methods to begin the registration process. Hover over the Log In or Register section and click Register. Or, click MY PHLY at the top of the page. The Log In to MYPHLY page appears. Click Create a new account.

The My PHLY Registration page appears. Select the radio button next to Insured. The registration page is displayed. 1. Enter the Account Number listed on the most recent invoice.

If you do not know the Account Number, click the hyperlink next to the Account Number field, enter a few pieces of information, and it will be displayed. 2. Enter the Policy Number. 3. Enter the desired User Name to log in to My PHLY. You create this. 4. Enter the First Name. 5. Enter the Last Name. 6. Enter the Address. 7. Enter the desired Password that will be used to log in to My PHLY. You create this. Important Note: Passwords must meet the following criteria: Contain at least 7 characters (letters and symbols #, etc.) Contain at least 1 capital letter Contain at least 1 number Passwords must NOT reference the User Name in any way. 1. For example: User Name: JoeSmith Password: JoeSmith123 or Smith4321 will NOT work Click the About Passwords link to learn more information about password format. 8. Confirm the desired Password by entering it again in the Confirm Password field. 9. Click Continue.

3 10. On the next page, re-enter the password you just created and set up a password reset question. If you forget your password, we will ask you this security question and you will enter the answer you have selected. Please note that your answer is case sensitive. Registration is completed - The My PHLY home page appears: From the My PHLY home page, Insured s have the ability to access claims searches and report claims, access online bill pay, view and download policy documents, access our Risk Management services, update preferences, and obtain quotes on select policy lines. CLAIMS SEARCH - REPORT CLAIMS From the left navigation bar, click Claims Search. If any claims had previously been filed, they would be listed on this page. On this page, the Report a Claim function is available to start the process of reporting a new claim.

4 Click on the Claim Type associated with the new claim to open and fill out the form. ONLINE BILL PAY From the left navigation bar, click Online Bill Pay to view and pay the current balance on the account, sign up for ebill, and sign up for recurring payments.

5 Enroll in Recurring Payments: 1. Click Online Bill Pay. A pop-up window will appear. Note: Your browser may prompt you to change your pop-up settings before continuing to online bill pay. 2. Click My Payments on the left menu. 3. Click Add Bank Account. 4. Add Bank Account information. 5. Click Add. 6. Click Recurring Payments Tab. 7. Click Add next to the Payment Timing Field. 8. Choose to pay your bill when the bill is received or when the payment is due. Select your payment account preference and click Add.

6 Enroll in ebill: 1. While in Online Bill Pay, click My Profile. 2. Click My Accounts. 3. Select the check box to the left of the account and click Edit. 4. Select Yes to the pop up. *Invoices will be ed to the address we have on file under Preferences. VIEW POLICIES AND BONDS: From the left navigation bar, click Policies & Bonds to perform a search by Policy Number or Account Name. By clicking the Blue hyperlink Policy Number you will be directed to the Policy Summary screen. On the Policy Summary screen you can download policy documents, endorsements, cancelation/reinstatement notices, and view billing/payment information. POLICY SUMMARY

7 RISK MANAGEMENT SERVICES A variety of risk management services can be utlizied to reduce business losses. Offerings include access to Risk Management Programs, educational e-flyers, and information on large claim loss lessons learned. PREFERENCES This section will provide identifying information, as well as, several actions that can be taken. By utilizing the drop down next to I would like to, various actions can be performed: Use the short cut to sign up for ebill using DocuSign.
