Reason: Merger Condition April 12, Title: FCC Triennial - Batch Hot Cut Process West (C /385659) Description: This initiative allows CLECs to request two new types of hot cuts, Large Job and Batch Hot Cuts. A Large Job hot cut applies to end-users with 20 or more loops at a single address. A Batch Hot Cut can be requested for a single loop; this single loop will be batched with other hot cut loops on a Central Office basis for simultaneous provisioning. Both Large Job and Batch Hot Cut require CLECs to use the Wholesale Provisioning Tracking Systems (WPTS). 2 02/10/04 Business Rule changes include two new values on the Coordinate Hot Cut (CHC) field of the LSR form when ordering either Large Job or Batch Hot Cuts. April 12, Title: Full Automation of Jeopardies - Phase II - Part 2 (C /379741) Description: In May 2003, CR C Jeopardy Remarks was submitted. This 5 CR stated that, in January 2003, Verizon West initiated changes to automate Jeopardy responses to the CLEC. Until that time, responses included remarks from the Verizon department placing the order in jeopardy and these remarks gave specifics on why the order was in jeopardy. Automation eliminated the remarks on the jeopardy responses to the CLEC. The CR requested that the jeopardy responses again include these remarks. The CR also requested that remarks include information about No Facilities jeopardies. 4 02/10/04 C Full Automation of Jeopardies Phase II, Part 2 will satisfy the requirements of C Jeopardy remarks will again be reported. The LR RDET and the LR REMARKS fields will be used. The field length for LR DET is 60 characters and the field length for LR REMARKS is 400 characters. Availability of facilities will also be included as before. Verizon reports 1) if there are no facilities; 2) If there are facilities; and 3) when facilities have been ordered by the CLEC, a projected completion date. Comments: This initiative satisfies the requirements of a 5 CLEC Initiated CR: C / T5 Jeopardy Remarks. March 16, 4

5 June, Place Holder Title: Connectivity Place Holder Description: This is not a CR. It is merely a Place Holder for any Connectivity issues that might be implemented with the June Release. LSOG Version: N/A Documentation Impact: N/A Title: Provider Notification Report will report losses when a CLEC migrates from Wireline to Wireless (C /384467) 4 02/13/04 Description: This initiative modifies the existing Provider Notification Report so that Wireless is uniquely identified, and can be distinguished from Wireline Loss Notification. The existing Provider Notification Report will include a new indicator specifically for Wireless. This new indicator will help the CLECs easily identify Wireless from Wireline for Loss Notification. New Value: PN will populate W in the NT field of PN form if NPDI=C for Wireline to Wireless conversion. LSOG5 and LSOG6 Form name: PN Field name: NT (Notification ) Field number: 001 Valid Value: W Valid Entries: Wireless LNP Edits/Usage Rules: When NPDI=C, W=Wireless LNP Comments: Rating 3.3 Title: T5 Retain Directory Listing on Loop Disconnect Orders (C /384095) Description: This initiative retains the directory listing for accounts where the CLEC is disconnecting the Verizon Loop and wishes to retain the Fictitious Listing. The scope of this initiative is to expand the use of the EU ERL/ELT field to provide the option for the CLEC to retain the listing as-is for all AB/D (Loop disconnects). This 5 02/13/04 March 16, 5

Reason: Enhance Flow Through, Measure 4 in CA, and 501/503 FCC Comments: Title: T5 CSI End User Migrations for Loop and Line Share Accounts (C /378294) Description: This initiative will allow the processing of CSI transactions that will provide the New Local Service Provider (NLSP) the ability to submit CSI Inquiries by billing telephone number (BTN) to Verizon. Reason: 5 CLEC Request 5 Comments: Rating 4.2 Title: Fiber to the End User for Wholesale (C /384810) Description: Verizon will provide Wholesale voice service in areas served by Fiber to the end user. Fiber will be deployed in two primary market environments: Overlay and Greenfield. In an Overlay environment, Verizon will deploy fiber facilities alongside existing copper facilities in selected wire centers. In the Greenfield scenario, fiber will be deployed instead of copper where no facilities exist. Verizon is not currently required to provide unbundled service in Greenfield environments. 4 Reason: New Product Comments: Documentation Impact: LSOG5, LSOG6 and Trouble Administration March 16, 7

8 Title: Change Number of Allowable Characters in LD1, LD2 and LD3 Fields (C /384277) Description: This initiative expands all the LD1, LD2 and LD3 fields on Order, Pre-Order from 4 to 10 alpha characters. For CSI, LD1, LD2, and LD3 the field will be expanded to 10 alphanumeric characters to accommodate the embedded base. 1 Reason: Comments: Trouble Ticket Title: T5 Hunt Group at Account Level P2 (C /383857) Description: This initiative is to address the embedded base issue where Hunt Groups and/or Sequences are not populated on Customer Profile, in addition to correcting the Hunting IOSCs to match with the appropriate Hunting Telephone Numbers. This initiative will also identify the owner of any manual effort for clean-up. 5 Reason: 5 CLEC Request Comments: Rating 4.4 Title: T5 Provider Notification in WISE for All Products (C /383826) Description: This initiative will allow access to Provider Notification reports in WISE for any Product, allowing an alternative transport medium when EDI or FAX problems arise. 5 Reason: 5 CLEC Request Comments: Rating 4.5 March 16, 8

9 Title: vrepair West TAS Replacement (C /384062) Description: Move the West Trouble Administrative System over to the East vrepair system and to use the East LSI-TA (Local Service Interface-Trouble Administration). This will be implemented in phases through. 4 Reason: To create a single service assurance management system for the entire Verizon footprint. Comments: LSOG Version: N/A Documentation Impact: Business Rules Trouble Administration Title: FCC Triennial - Batch Hot Cut Process Phase 2 (C /385659) Description: Batch Hot Cut orders to process within Flow Through. Reason: The scope of this initiative is to respond to the FCC guidelines to (FCC 03-36) 2 Comments: New Standardized Manual Edits Form Section Field Error Code LSR Administration CHC E05/E06 FLDH99830 LSR Administration CHC E05/E06 FLDH99829 LSR Administration SUP E05/E06 FLDH99828 Edit Message Non-WPTS CLECs cannot order Large Job or Batch hot cuts. If DFDT is populated and CHC is blank, CCNA must not be listed on WPTS user. If SUP = 5, existing NOCV order cannot be a Batch hot cut. Title: FCC Triennial Network Modifications No Build UNE Loops (C /385106) Description: The purpose of this initiative is to provide to CLECs that have signed a TRO agreement, notification that there is no compatible facilities available from their request for new wholesale services i.e. UNE-P, UNE Loops etc. The CLEC will have the ability to request a cost estimate for work activities needed to fulfill their March 16, 9 2

In addition, processes shall be modified to afford the CLEC the opportunity to cancel or authorize the estimated work activities provided from the cost estimate request and be billed for these services. Reason: FCC Triennial Review Order (Designation as 2 Under Review) Comments: Title: T5 Shared UNE Loop Product West (C /384693) Description: This initiative will allow Verizon to accept an LSR that requests Loop Sharing migration orders. The Loop Sharing arrangement should happen in a fashion similar to that in place for line splitting. *5 In addition to the creation of the Loop Share Product this initiative also incorporates three new Loop Share Migration LSRs: --Line split (UNE-P CLEC Voice/CLEC ) to Loop share Migration (CLEC Voice/CLEC ) w/w/out NP) --Line share (VZ Voice/CLEC ) to Loop share Migration (CLEC Voice/CLEC / w/w/out NP) --Loop share (CLEC Voice/CLEC ) to Loop share Migration Reason: 5 CLEC Request *This initiative incorporates the following three 5 CRs: C /386120, C /386119, and C / CR # Init # Description C From Line Splitting (UNE P) to Loop Sharing Assumptions: Existing Arrangement CLEC UNE P Voice; DLEC provider in Line Splitting arrangement New Arrangement New CLEC Voice on xdsl capable UNE Loop; no change in DLEC data provider CLEC & DLEC partnership agreement, pre-positioned 30 business days prior to order activity.
