Lunch Rego vi your Ptient Record System Open the Rego referrl templte vi your Ptient Record System: Exct / SOE: Open the ptient record, then click on Ptient documents, locte the Rego Referrl nd click Preview R4: From the menu r, select Tools > Refer Ptient Out > Click on Send Letter nd select the Rego referrl ) Click on the Rego utton Note: in some configurtions, Rego my lunch utomticlly fter you hve selected the referrl templte. If Rego hs lunched utomticlly, continue to step 2

3 2. Access Rego ) Prctice nme nd Ptient Surnme will e displyed ) Select User c) Select Referring Clinicin (if different to user) c 3. Medicl History nd Ptient Modifiers ) Enter ny current medicl conditions for the ptient. E.g. Dietes ) Add ny medictions the ptient is on tht my increse the risk of leeding. E.g. nticogulnts c) Enter ny other ptient ssocited modifiers. E.g. Ptient is in Wheelchir d) Enter if the ptient hs either sever gg reflex or severe dentl nxiety. e) Enter ny other informtion out the medicl history nd ptient modifiers in the free text ox provided. (Tht cn e left lnk if no other informtion is required. c d e

4 f) Select whether you would like to uplod ny dditionl informtion, this is not mndtory. g) If selected yes on previous question, select the Add option tht hs ppered nd you cn uplod dditionl documents. E.g. Comprehensive lists of medicl conditions nd medictions, if required. f g h) Select the Document t nd dd ny dditionl/ccompnying documents should you need to. i) When tht the section is complete select the Next utton in the ottom right. h i

Selecting the Pthwy ) Now you hve to select wht it is you wnt to refer the ptient for, whether it e nd extrction, orthodontics, two week wit referrl (suspected cncer), TMJ etc You cn select more thn one pthwy for ech ptient, so if ptient requires oth n extrction nd two week wit referrl (suspected cncer), you cn send oth of those referrls simultneously if needed. ) Once you hve selected the required pthwy(s), click Next. 5. The Mnnequin ) Select the teeth required for extrction, once selected they will highlight in purple. ) +For ech tooth selected you will e sked to input reson for extrction nd n ssocited complexity use the drop down lists provided to select the relevnt options, these options were developed y pthwy review group nd re viewed y tht group s ll resons for referrl in ech drop down ox.

6 c) Specify whether you hve ttempted the extrction lredy. d) If yes to hving previously ttempted the extrction, plese provide ny supplementry informtion out tht extrction if needed. e) Specify whether the ptient is undergoing privte course of tretment. f) Select the dd utton to dd pproprite rdiogrphs to the referrl c d e f g) Use the first t X-ry/Imge from file to dd X-rys if your prctice hs digitl X-rys. h) If you do not hve digitl X-rys nd were provided with PA Scnner, plese use the second t PA/ Introrl imge nd ttch the device to the PC vi USB, plese see the PA Scnner guide if you need help with this. i) You cn lso follow steps 1-5 on the From Moile t to dd ny imges you were unle to get n X-ry of, for exmple white ptches when sending suspected cncer referrl, or fcil lesions. Plese Note: using the from moile function if you re to tke the pictures with the ptient there nd then it will delete n imges you tke from your phone to e in ccordnce with dt protection nd ny other legl requirements.

7 j) On Ech imge you dd you will hve to enter dte of exposure, either y clicking tody or typing in dte. k) You hve n option to dd ny notes out imges you hve. j k l) When tht the section is complete select the Next utton in the ottom right. l 6. Ptient Choice ) The system will trige the referrl to either primry or secondry cr sed on the complexity of the cse. This section will tell you were it s triged the referrl s the most relevnt list will e the one tht ppers on top. If you feel the referrl needs to e seen t hospitl when it hs een triged to primry cre, the primry cre prctice will hve to review the vse nd gree to e le to send it onto hospitl, otherwise they will proceed with tretment if they deem it possile to do so. ) You cn see the list of primry cre providers c) You cn see n pproximte wit time for n ppointment d) You see n pproximte wit time for tretment fter the ppointment e) You lso see the distnce of the ech prctice from the ptients home so s to mke the most informed decision s to where to send the ptient. f) Select one option from ech list you re given, they will highlight when you do so, selecting more thn one option will occur if the referrl is triged to primry cre nd so you will e required to select secondry cre option incse the referrl needs to e esclted.

8 e c d f 7. Consent ) Lst thing is to confirm the ptient hs given their consent to sending the referrl. ) Finlly, click Send to rin g up the confirmtion screen nd to complete the referrl.
