1 General Instructions to the Bidders of AMC for PCs & Peripherals for Unit AMC cost should be indicated separately for each PC / Peripheral. Annexure-1 2. All the PCs, Printers and Scanners listed out in Annexure-2 requiring AMC service are in good working condition. 3. Bidders are requested to go through the Scope, Terms & Conditions of the AMC, enclosed as Annexure-3 herewith thoroughly and completely, before submitting the quotation. 4. Offer should be clear in all respects, without any ambiguity. 5. The probable date of commencement of the AMC is Tentative list is attached in the annexure. 6. Madras Atomic Power Station, being an organisation of Nuclear Power Corporation, coming under the Energy sector, engaged in the production of electricity round the clock the service required for the PCs and Peripherals of this AMC should be the excellent and prompt and with priority. 7. Certificates to be enclosed along with the offer: The firm who are new to NPC, in offering such AMC service, shall submit satisfactory service certificates from at least three Govt. / PSU organisations for having provided such AMC service. The certificate shall contain no. of PCs / Peripherals covered in AMC and name, designation, contact phone nos. of the signatory of the certificate. 8. Suppliers who have been doing or doing at present such AMC work in any of the constituent unit of NPC / DAE within the country must furnish details of the same, enclosing copies of order, certificate of satisfactory service. * * * Page 1 of 10

4 Annexure-3 SCOPE, TERMS & CONDITIONS OF THE AMC In what follows, " AMC " - means Annual Maintenance-service Contract for PCs and peripherals available at three places: MAPS Site-Kalpakkam, NPCIL office Chennai-8 and ES Lab Township-Kalpakkam. " Supplier "- means the supplier who will be providing maintenance service in the event of AMC order " ENC " - means the Engineer-in-charge, IT&C, MAPS, Kalpakkam, and " NPC " - the Nuclear Power Corporation (I) Ltd., Kalpakkam / Chennai CONTRACT PERIOD : The period of AMC shall be ONE year. It is extendable mutually further, subject to the terms & conditions of NPC as applicable at that time and satisfactory service in the past RATE FIRMNESS : The AMC charges quoted must be firm throughout the contract and no escalation of any kind is permissible. period 3.0. SCOPE OF AMC : The service of AMC shall be comprehensive in nature, covering both Break-down Maintenance and periodical Preventive Maintenance (PM) for all the PCs & peripherals covered in the AMC, Maintenance of existing network, connectivity checking and rectification of nodes, crimping etc COMPONENTS COVERED : AMC shall cover all the components of PCs and peripherals including Mother board and other connected boards, ports, hard disks, RAM, CD &DVD Drives, floppy drives, picture tubes, panels, keyboards, Speakers, Modems, Mice, adaptors, print-heads of Printers, Teflon sheets of printers, bulbs of scanners and batteries for PC clocks.,lan CARD and excluding the inkjet cartridges, printer ribbons toners of laser printers, and network components No additional charges is payable towards cost of spares covered in 3.1. 5 3.3. MAX.DOWNTIME PERMITTED: Break down problems shall be rectified within 24 hrs. of intimation of problem at MAPS site / NPC Chennai office, latest by the immediate following NPCIL working day. Downtime beyond 24 hrs., for an equipment is not permissible. Please see condition sl.no. 6.3 on downtime RESIDENT SERVICE PERSONNEL FOR KALPAKKAM: Supplier shall provide Resident service personnel that at least SEVEN service personnel shall be visiting MAPS site, Kalpakkam on daily basis from 0915 to 1715 hrs on all NPC working days. The supplier has to make his own arrangements of accommodation for his personnel to stay nearby place of MAPS, for maintaining timing discipline Among the Seven service personnel, one shall have knowledge to assist the development activities of IT and to take care of call allocation, peripherals movement, record maintenance, etc., one person shall be knowledgeable to solve the maintenance of printer and Monitor at site itself, one person should take care of calls pertaining to township and Chennai office and to support other meetings, and the other 4 for PC & other peripherals maintenance at MAPS. The supplier shall have to provide qualified and experienced service personnel for the above maintenance and shall not change the service personnel unless or otherwise insisted by MAPS. In case of any personnel allotted for MAPS is unable to attend duty due to various reasons such as resignation or sick for a longer period, the supplier shall have to provide alternative personnel for performing the services. In case of any changeover of staff is warranted, there should be a minimum of one week required for both staff for proper changeover. Supplier shall also provide additional man power,if required, for PM, and equipment specialists (like plotter, monitor, printer, server etc.) to clear specific problems. The supplier shall keep additional man power to be available in view of occasional leave of their staff, to meet required minimum man power,to avoid penalty towards absenteeism. The supplier shall have to make his own conveyance to take care of material movement and the calls pertaining to township and Chennai STOCK OF READY & GOOD SPARES: Supplier shall maintain sufficient qty. of essential spares like: PC Mother boards, hard disk / floppy / CDROM drives, RAMs, Monitors, SMPS, interface cards, Keyboards, Mice and Printers of all types at MAPS site and covered under this AMC. This is to provide immediate replacement or substitute that compliance to condition 3.3. is strictly maintained. The supplier shall have to provide one PC exclusively for the service personnel for their record maintenance and other testing works NO REPLACEMENT WITH LOWER TYPE COMPONENTs: Malfunctioning components should be replaced with equivalent or better components. This means, if an hard disk of 160 GB size breaks down and could not be restored it online within the stipulated time, the replacement should be of same 160 GB size or could be more, but not of lesser size. 6 3.7. STANDBY ARRANGEMENT: When supplier feels servicing an NPC's faulty component / equipment may take time and so installs a higher type and brand new item in its place, as a standby arrangement that his item should be returned to him once the original NPC's item is installed back, it should be with the concurrence of ENC. Such standby arrangement is allowed, only in instances where: a 160 GB HDD is replaced with 200 GB size or a mono monitor is with SVGA color monitor from supplier's account CONTROL OF STANDBY ARRANGEMENTS: All standby arrangements falling under condition 3.7. should be recorded separately and maintained. The permissible time limit, that such an arrangement may continue is one month or upto the end of that AMC quarter whichever is earlier. Cases where this time limit is too short will be treated suitably PREVENTIVE MAINTENANCE (PM): PM shall be done once in 3 months, for all the eqpts. of AMC. This shall include checking hard disks for good working condition & virus infection, identification and rectification of hard disk problems, head cleaning of floppy / CDROM / CTD drives, blowing out dusts from eqpts. using suitable vacuum cleaners, applying lubrication to Printer components, cleaning of print-heads / carriages of dot matrix/inkjet/laser printers and scanners, cleaning of cabinets / covers/ keyboards/monitors etc. with cleanex solution. The contractor should employ additional man power, if required,to complete the preventive maintenance in time. 5.0 PREVENTIVE MAINTENANCE STICKER : The supplier shall arrange stickers showing the AMC No, Inspection Details, PM done details etc as specified in the annexure-4 which have to be pasted on each equipment and the service engineers have to record the details of specific events on the sticker in addition to History Card Recording OTHER TERMS & CONDITIONS: 6.1. No extra charge is payable for to and fro, board and lodging for the service personnel coming to Kalpakkam / NPC Chennai Carrying NPC eqpt. by supplier for servicing at his premises will be allowed when it is really required so and allowed only on provision of substitute equipment as specified in condition sl.no.3.7. The NPC equipment after servicing should be returned at the earliest. Such carrying/returning of equipments is at supplier's risk. Page 6 of 10

7 6.3. (A)PENALTY FOR DOWNTIME BEYOND PERMISSIBLE LIMIT: FOR EQUIPMENTS OTHER THAN ONLINE EQUIPMENTS If an equipment is down for more than 24 hours, without any substitute as stipulated in condition sl.no above, the complete time will be considered as downtime. An equipment not fully functional is treated as down. (i.e., If a FDD not working, computer is considered to be down). For each occasion of such occurrences, the penalty for such down time is 2% of the equipment s AMC charge, per day, for the 1 st 7 days and 5% ot the equipment s AMC charge, per day, from the 8 th day onwards and penalty shall be deducted from the amount payable to the supplier. (B) FOR ONLINE EQUIPMENTS (SERVERS & INDUSTRIAL PCS) If an equipment is down for more than 8 hours, without any substitute as stipulated in condition sl.no above, the complete time will be considered as downtime. An equipment not fully functional is treated as down. For each occasion of such occurrences, the penalty for such down time is 5% of the equipment s AMC charge, per day, for the 1 st 7 days and 10% of the equipment s AMC charge, per day, from the 8 th day onwards and penalty shall be deducted from the amount payable to the supplier IMPORTANT NOTE:- Downtime penalty will be treated as the main criteria for deciding the continuation of the contract. Especially, the online equipments need immediate care and downtime will be viewed seriously for these equipments. If the downtime penalty is beyond a permissible limit, the contract will be terminated immediately. Hence the service provider shall take utmost care for maintaining required substitutes and spares for immediate replacement and put back the rectified equipments within a minimal time to avoid down time penalty and contract termination. (C) FOR ABSENTEEISM As per the clause 3.4, the Supplier shall provide at least Seven Resident service personnel for attending problems in MAPS site, Kalpakkam on daily basis from 0915 to 1715 hrs on all NPCIL working days. During the Billing month (between 19 th day of current month and 18 th day of next month), at the most 7 man days of absenteeism is permissible without any penalty. Beyond which a penalty of Rs. 1000/- (One Thousand only) per absent man day will be deducted from the amount payable. For example, during the billing month if the number of absent man days is 10, penalty for 3 (i.e. 10-7) man days will be applied This AMC shall be executed by the supplier and should not be sub-contracted to any other, in any form. 8 6.5. The equipments of this AMC shall be serviced only by the suppliers' authorised, qualified and competent service personnel MATERIALS MOVEMENT CONTROL: All the supplier's ready spares/items mentioned in sl.no. 3.5, brought inside MAPS will be identified by pasting a sticker titled as "AMC Spare" and control sl.no. with ENC's initial and date. This shall be maintained in a register CATEGORISATION OF OUTGOING MATERIAL: Any item issued to supplier within the AMC period for taking out of MAPS shall be categorised as follows:- (a) RETURNABLE :- NPC item falling under condition sl.no or supplier's item if the same one will be brought back after servicing at his premises outside MAPS. (b) NON-RETURNABLE :- Unserviceable supplier's item or NPC's item for which replacement as specified in condition sl.no. 3.6 has been provided and installed RELEASE OF NON-RETURNABLE ITEMS TO SUPPLIER : Only the items categorised as "NON-RETURNABLE" as specified in 6.7. (b) above will become supplier's property. These will be issued to the supplier as "Non-Returnable". Also, at the time of end of AMC, whatever supplier's items remaining as per records at MAPS, will be released to him as "Non-Returnable". 6.9 Safety,Insurance etc. The supplier shall cover the service personnel deputed for the above job for the period of contract under necessary workmen safety insurance scheme. All the service personnel deputed for the above job should strictly adhere to safety rules of MAPS and deviation or violation may attract penalty by respective agency 7.0. TERMS OF AMC PAYMENT: The AMC payment shall be payable in four equal instalments, at end of each quarter, provided there are no pending problems pertaining to that quarter. From this, the amount at the prevailing rate, which is subject to change as imposed by MAPS/NPC, per head per day of visit of all the personnel from the Supplier s firm to MAPS in that quarter will be deducted towards MAPS-canteen facility charges, regardless of use or non-use of said canteen by the Supplier s staff.. Also the penalty towards downtime will be deducted from the payment due for each quarter. Page 8 of 10

9 8.0. TERMINATION OF AMC: If the service support is found to be not satisfactory, the AMC will be terminated at the earliest. Downtime penalty will be considered seriously for contract continuation as it is the main factor for gauging service efficiency as mentioned in the clause 6.3. Providing efficient and regular service personnel will also be considered as another factor for deciding the continuation of the contract. If the personnel are irregular or inefficient the contract will be terminated WITHDRAWAL OF EQPTs FROM AMC: It is possible some obsolete PC / Printer/scanner may be withdrawn from AMC (by MAPS), during contract period. For such eqpts., the AMC payment will be done till the time it existed in AMC as shown below: No.of months in AMC x AMC Charge of eqpt. 12 The supplier shall not be allowed to withdraw any equipment from the AMC on his own due to any reasons. If the supplier is unable to service any equipment within a reasonable time or service for that equipment is not satisfactory, MAPS has the right to get the equipment serviced through a third party and the charges will be deducted from the payment due for the supplier. If any equipment becomes unserviceable due to non availability of spares, the supplier shall have to produce a valid proof for the same from the original manufacturer of that equipment and the same may be replaced with a better equipment on mutually agreeable basis ADDITION OF EQPTs. TO AMC: Depending on the quality of service provided for the existing equipments, additional PC and peripherals may be included in the AMC. The AMC charge of such additional eqpt. shall be calculated proportionally for the number of months covered as given below : No. of months to be covered x per month maint.charge of equivalent or nearest type eqpt. already present in AMC If the same category/model of the equipment is not available in the AMC list, rate of nearly equivalent item will be considered for the above purpose CERTIFICATES TO BE ENCLOSED ALONG WITH THE OFFER: The firms who are new to NPC, in offering such AMC shall submit satisfactory service certificates from atleast THREE Govt. / PSU organisations for having provided such AMC service. The certificate shall contain no. of PCs / peripherals covered in AMC and name, designation, contact phone nos. of the signatory of the certificate. * * * Page 9 of 10
